Arthroscopic Treatment of Gouty Tophi Causing Acute Impingement Syndrome of Bilateral Shoulder Joint: A Case Report

A poor compliant patient presented with gouty tophi in the bilateral glenohumeral joints, rotator cuff tendons, and subacromial spaces causing severe shoulder impingement syndrome and was treated with arthroscopic surgery after failed medical treatments. Postoperative patient education, compulsive diet and medical control program were given. Both subjective and objective satisfactory results have been achieved. Although rare, gouty tophi can be a source of severe shoulder pain and the cause of shoulder impingement syndrome. Arthroscopic decompression surgery can provide good symptom relief and improved functional outcomes. Symptom free gout control for tophi-induced shoulder impingement syndrome still depends on a meticulous diet, medical control, and postoperative rehabilitation.
