高雄市政府消防局實施到院前十二導程心電圖判讀措施成效探討－以休閒場域為例
Discussion on the effectiveness of the pre-hospital 12-lead ECG interpretation measures implemented by the Fire Bureau, Kaohsiung City Government- Take the leisure field as an example
摘要
心臟疾病為國人十大死因第二位，急性心肌梗塞病患若能夠在90分鐘內接受並完成心導管手術，將可以有效的降低患者的死亡率。高雄市政府消防局與衛生局、高雄榮民總醫院共同推動到院前12導程心電圖判讀措施，將心電圖判讀結果提前在現場即可完成，並提早預警給後送之醫院同時啟動心導管室，藉以提升患者存活率以及預後狀況。本研究以進行深度訪談以及文獻探討等方式，探討到院前12導程心電圖判讀措施可以自高雄市開始實施並順利推行的成功因素為何？推行時施面對的阻力及困難如何克服解決？於休閒場域執行此措施有何成效？並綜合分析資料後，對到院前12導程心電圖判讀措施提出四點建議：一、到院前十二導程心電圖機硬體改進之建議。二、心電圖相關教育訓練之建議。三、緊急救護工作專責化之建議。四、到院前十二導程心電圖判讀措施相關宣導之建議。

Heart disease is the second leading cause of death among Taiwan people, if patients with acute myocardial infarction can receive and complete cardiac catheterization within 90 minutes, it will effectively reduce the mortality of patients. The Kaohsiung City Government Fire Department, the Health Bureau, and Kaohsiung Rongmin General Hospital carried out a cooperation between civil and government departments to jointly promote the pre-hospital 12-lead ECG interpretation measures ,The ECG interpretation results can be made in advance. Complete, and give early warning to the evacuation hospital to start the cardiac catheterization room at the same time in order to improve the patient's survival rate and prognosis. In this study, we conducted in-depth interviews and literature discussed to discusse why the pre-hospital 12-lead ECG interpretation measures can be implemented and implemented successfully in Kaohsiung City, how to overcome the resistance and difficulties during the implementation, and the effect of implementing this measure in the leisure field? After comprehensive analysis of the data, the study made four suggestions: 1. Suggestions of hardware improvement of the pre-hospital 12-lead ECG machine. 2. Suggestions of ECG related education and training. 3. Suggestions of specialization of emergency ambulance work. 4. Suggestions of propaganda about the pre-hospital 12-lead ECG interpretation measures.
