超音波影像評估腹橫肌厚度變化之探討

摘要


由於現代生活模式的改變,導致脊椎不穩定、肌力下降或動作控制不良所造成的下背痛問題日漸普遍,因此,腰部肌肉功能障礙成為許多研究的重點。研究發現藉由訓練腹橫肌運動收縮,強化脊椎節段穩定性,可顯著改善下背痛的復發率。在復健計劃中運用超音波影像評估腹橫肌變化,透過即時影像回饋可提供腹部核心訓練的參考依據。本研究以文獻回顧法,蒐集國內外文獻之研究結果,綜合獲得以下結論:一、超音波影像技術因檢查前準備作業少,相對節省所需費用,且降低曝露電離輻射量,可提供動靜態即時影像;另評估肌肉厚度之信效度佳,故可用來評估腹橫肌的收縮變化。二、近年來醫療體系逐漸增加使用超音波影像技術,用於評估腹部深層肌肉的結構、核心訓練或復健操作效果。三、日常生活中應持續保持腰骨盆中立且腹部收縮姿勢,更能提升腹橫肌積極參與鞏固姿勢效益,達成維持脊柱穩定的功效。

Background: Low back pain, caused by spinal instability, decreased muscle strength and poor motor control problem, has become more and more popular in primary care clinics, resulting in more and more research focused on muscle dysfunction of the lumbar spine. It has been noted that different contraction types may affect the stability of the spine and increase the strength of the transverse abdominis by segmental stabilization exercises may significantly prevent recurrence of low back pain. The ultrasonography has been used to measure changes in the transverse abdominal muscle directly during contraction and to evaluate the effectiveness of the rehabilitation program. The immediate feedback provided by ultrasound images gives more insights into the abdominal core training. Conclusions: 1. Ultrasonography is feasible to measure thickness of the transverse abdominis based on the features of less preparation before operation, inexpensive, less exposure to ionizing radiation, providing statics/ dynamic real-time images, high validity and reliability in meaurement of muscle thickness. 2. In recent years, the applications of ultrasonography to examine the abdominal muscle structures among patient groups, to evaluate the effects of the core strength training or rehabilitation programs became more popular. 3. Researchers have suggested that to maintain a neutral lumbar/pelvic position and abdominal contraction posture during daily life enhance active contraction of the transverse abdominis providing more stabilization of the spine.
