This report describes a rare case of testicular cavernous hemangioma. A 34-year-old man presented painless right testicular swelling that had lasted for three months. All tumor markers were negative. Scrotal sonography revealed a heterogenous mass at the lower portion of the right testis with no increased vascularity in the color Doppler scan. An intraoperative frozen tumor section revealed a benign vascular tumor, for which a testis-sparing operation was performed. The pathological report showed a testicular cavernous hemangioma. Testicular cavernous hemangioma is a rare but benign lesion which should not be considered as a malignancy. A testis-sparing operation is considered adequate treatment.
