自行中斷美沙冬治療患者之新生兒發展評估個案報告
Evaluation the Development of a Child whose Mother was Withdrawing from Methadone Maintenance Treatment during Pregnancy
摘要
過去研究指出，濫用鴉片類藥物的婦女，其新生兒有發展遲緩的傾向，但台灣對於這方面的研究卻付之闕如，本研究為個案報告，此個案在懷孕過程中，由於案母擔心美沙冬的影響，自行退出替代治療，但又斷續使用海洛因及其他藥物。生產完後案母帶個案前來兒童精神科就診評估，我們使用貝莉氏嬰幼兒發展量表第三版進行評估，發現個案語言有發展遲緩的現象。由此個案觀察，沒有規則接受美沙冬替代療法治療的孕婦，懷孕過程有其他藥物濫用的風險，亦有有幼兒發展遲緩的風險，但由於此為單一個案報告，仍需要其他相關研究的佐證。

Although the delay in the development of babies born to mothers abusing opioids during pregnancy has been described in several reports, only recently has this issue been noticed in Taiwan. This case report describes a development of a child with a mother withdrawing from Methadone Maintenance Treatment (MMT) program during pregnancy. The mother, withdrawing from MMT, self-medicated using harmful substances such as heroin during pregnancy. These harmful chemicals may have interfered with a baby's healthy development. Based on the Bayley scales of infant development, her child had marked developmental delay, a finding consistent with previous studies. Description of the case is followed by a discussion of the limitations of this case report and suggestions for future prospective research.
