目的:以健康行為模式為框架,探討傷口照護門診病人選擇自費敷料的因素。方法:我們在新北市的一家地區醫院進行了問卷調查。問卷參考了過去的研究,並依照健康行為模型的架構進行設計。結果:本研究募集50名病人,個人月收入以無收入與新台幣26,400元至44,400元各佔32.0%。另外,「整體使能因素」平均得分為4.18±0.60。而「整體需求因素」平均得分為4.39±0.53。個案對於自費敷料願意接受的價格以新台幣「1-499元」佔42.0%。最後利用二元羅吉斯迴歸分析。結果顯示,在個人月收入新台幣44,401元以上的病人,接受自費價格新台幣1,000元以上的可能性,顯著高於收入在26,400元以下者(勝算比=31.57,p值=0.019)。結論:健康行為模式中前傾因素的個人月收入與傷口照護門診病人選擇自費敷料有關。本研究的發現用於自費敷料的推廣。未來的研究應探討如何透過措施,如:資訊公開或是醫病溝通以促進自費敷料的使用。
Objectives. To evaluate the factors affecting the use of self-pay dressings for outpatient wound care by using a health behavior model. Methods. We conducted a questionnaire survey at a regional hospital in New Taipei City. The questionnaire was designed with reference to questionnaires in previous studies and in accordance with the framework of the health behavior model. Results. This study enrolled 50 patients. Regarding personal monthly income, no income and an income of NTD$26,400 to NTD$44,400 were the two major categories, with 32.0%. of participants falling in each category. Additionally, the average score for Overall Enabling Factors was 4.18, with a standard deviation of 0.60. The average score for Overall Need Factors was 4.39, with a standard deviation of 0.53. Regarding the acceptable price for self-pay dressings, the largest portion of participants (42%) believed an acceptable price to be NTD$1 to NTD$499. Finally, the results of a binary logistic regression model revealed that the willingness to pay higher costs for self-pay dressings (NTD$1,000) was significantly higher among patients with a monthly income higher than NTD$44,401 than among patients with a monthly income of less than NTD$26,400 (odds ratio=31.57, p=0.019). Conclusion. Monthly income was a predisposing factor and was associated with a willingness to accept self-pay dressings. Our finding is useful for the self-pay dressing promotion. Future studies should apply strategies such as disclosure of information or doctor-patient communication to facilitate the use of self-pay dressings.
