糖尿病所致之動眼神經病變的針灸治癒病例
Cured Case Report of Oculomotor Neuropathy Treated by Acupuncture
摘要
這位58歲男性塑膠廠員工，在97年02月08日忽然左眼看不到，其症狀還包括：眼睛疼痛、頭暈，且在三天以後，左眼的上眼瞼下垂，完全無法張開，左眼的眼球移至眼角的外側部位，眼球也完全無法移動。在左眼發生問題之前，有糖尿病、高血壓，雖有服用降血壓和降血糖的西藥，但二者皆控制不好。患者的腎功能也不好，尿酸過高，並且在左眼看不見之前，有較嚴重的頭暈現象。在住院期期間，他有繼續接受服用降血壓、降血糖的藥物，和治療末梢神經的藥物並安排理學檢查、腦神經檢查、眼科的檢查，雖然有一連串的檢查與服藥，但症狀均未改善，因此住院8天之後，經由主住醫師評估出院，改為門診治療，並且到中醫部針灸科尋求治療（97年02月19日）。針刺穴位是：睛明穴、攢竹穴、瞳子穴、絲竹空穴、陽白穴、四白穴、足三里穴、三陰交穴、築賓穴、太衝穴、太谿穴。在持續針刺治療後，患者的病情漸漸明顯改善，約針刺將近二個月的時間，左眼的視力，眼球的移動及眼皮的張開都跟原來的一樣，只剩下左右眼在同時看物時，會出現些微複視的現象。

The 58 y/o male worker of a plastic factory complained about losing his left eyesight abruptly on 2008/02/08 with accompanying symptoms of oculoalgia, dizziness. Three days later he had ptosis of left upper blephar with difficulty in fully eye opening and external deviation of left eyeball and inability of eye movement. He had medical history of diabetes mellitus and hypertension prior the episode of ophthalmological problem without fair medical control of anti-hypertensive and anti-diabetes drugs. His renal function is not well with high serum level of uric acid, besides, he had severe vertigo before blindness.During his hospitalization, he continued to receive anti-hypertensive, anti-diabetes and anti-retinopathy drugs and was arranged to receive physical exam, cranial neural exam and ophthalmological exam. After a series of exams and medications, his symptoms were still not improved. Therefore, he was evaluated to be discharged from hospital after hospitalization for eight days by his attending doctor, and receive outpatient treatment of acupuncture at the Traditional Chinese Department of KMUH (Kaohsiung Medical University Hospital) since 2008/02/19.The acupoint he received are Jingming (BL 1), Cuanzhu (BL 2), Tongziliao (GB1), Sizhukong (SJ 23), Yangbai (GB14), Sìbái (ST 2), Zúsanl (ST 36), Sanyinjiao (SP 6), Zhubin (KI 9), Taichong (LR3), Taixi (KI 3). After continuously treatment of acupuncture, the symptoms of patient were improved. His eyesight, eyeball movement and blephar opening of left eye were recovered after acupuncture therapy for two months. Slightly diplopia of both eyes while looking object at the same time was complained after therapy.
