Radioiodine (I-131) is an established ablative treatment in patients with differentiated thyroid carcinoma, and I-131 whole-body scan is widely used in the clinical setting to detect functional metastases. Foci of non-physiological uptake are usually considered thyroid metastases but false-positive results due to non-thyroid tumors or humoral contamination have been reported. We report a case of incidental finding of I-131 activity in the patient’s oral cavity, which was localized by single photon emission computed tomography (SPECT)/computed tomography (CT) to the mercury dental filling. There is no evidence suggesting concomitant dental or periodontal disease. Hence, potential false-positive activity related to dental conditions should be treated with caution.
