Paragangliomas are rare tumors of neural crest origin that arise from the extra-adrenal autonomic paraganglia. A nonfunctioning paraganglioma could be discovered incidentally on imaging studies. Herein, we report the case of a 68-year-old man with early-stage lung cancer underwent a post-operation staging fluorine-18-fluorodeoxyglucose (F-18 FDG) positron emission tomography/computed tomography. A solitary well-defined nodule with extremely intense FDG was incidentally noted at the hepatogastric region. Finally, laparoscopic excision of the lesion was performed, and the diagnosis of malignant paraganglioma was confirmed by the pathological findings.
