Serum thyroglobulin (Tg) level is a biochemical marker that detects recurrence of differentiated thyroid cancer. Falsely elevated Tg levels may be influenced by other clinical conditions. A 69-year-old male was diagnosed with papillary thyroid carcinoma (PTC) post-total thyroidectomy and radioactive iodine ablation. Although his serum Tg level was below 0.2 ng/mL during long-term follow-up, it increased to 0.36 ng/mL two months after his second COVID-19 vaccination, with no evidence of recurrence. A 56-year-old female was diagnosed with PTC post-thyroidectomy and radioactive iodine ablation. Her Tg level increased to 0.3 ng/ mL one month after receiving each of two COVID-19 vaccinations, but without recurrence. Falsely elevated Tg levels were reported in two cases after they received COVID-19 vaccinations. Transient increases in Tg raise clinical suspicion of recurrent disease and require close observation for 3-6 months.
甲狀腺球蛋白是追蹤分化型甲狀腺癌是否復發的常用指標,然而一些特殊的臨床狀況也可能導致數值上升造成偽陽性。本次兩位案例分別為69歲男性及56歲女性,診斷為甲狀腺乳突癌經手術及放射碘治療,並於門診長期追蹤甲狀腺球蛋白數值均穩定。第一位個案於注射新冠肺炎疫苗兩個月後出現甲狀腺球蛋白暫時性上升,影像上無復發跡象,後續追蹤6個月甲狀腺球蛋白數值均維持小於0.2 ng/mL。第二位個案兩次注射新冠肺炎疫苗均於注射後一個月出現甲狀腺球蛋白暫時性上升,超音波顯示無復發跡象,後續門診追蹤6個月甲狀腺球蛋白數值均小於0.2 ng/mL。新冠疫苗注射後可能出現甲狀腺球蛋白暫時性上升情形,若影像上無復發跡象,可持續追蹤三至六個月排除偽陽性的可能。
