Gastrointestinal stromal tumor (GIST) is a rare neoplasm with nonspecific clinical manifestations, often incidentally discovered during surgery or through imaging studies. When presenting as a pelvic mass, it poses a particular challenge in the differential diagnosis of gynecological tumors. Here, we present a case discussing a gastrointestinal stromal tumor mimicking a gynecological tumor and provide a review based on the existing literature. As a professional obstetrician and gynecologist, it is crucial to be aware of such atypical presentations and consider GIST in the differential diagnosis when encountering pelvic masses.
