利用QuantiFERONÒ-TB Gold In-Tube (QFT-IT)其具有結核分枝桿菌專一性分泌蛋白ESAT-6、CFP-10和TB7.7刺激血液中T細胞釋放丙型干擾素(IFN-γ),以評估感染結核病之可能。檢體取自疑似接觸者人員之血液,再利用酵素免疫分析法(Enzyme-linked immunoassay, ELISA)檢測丙型干擾素濃度,進行潛伏性結核菌感染檢測並分析。結果:接觸者血液檢體共計6,753件,其中台北市衛生局1,165件、新北市衛生局所5,325件、台北市立聯合醫院255件、結核病高危險群醫檢人員8件。檢測後台北市衛生局檢體有122件陽性,陽性率占10.47%(122/1,165);新北市衛生局所檢體有1,018件陽性,陽性率占19.12%(1,018/5,325);台北市立聯合醫院檢體有45件陽性,陽性率占17.65%(45/255);結核病高危險群醫檢人員檢體有3件陽性,陽性率占37.50%(3/8);本次潛伏性結核菌感染患者為17.59%(1,188/6,753)。由數據顯示在廣泛定義的接觸者陽性率約為18%,而醫檢從事人員是陽性率最高的危險族群陽性率約為38%。藉由此探討,期許對疑似結核病接觸者的診斷有所依據,使其能夠早期發現早期治療。而對不同感染結核病之風險,暴露於較高危險之醫、護、檢族群應更加注意工作環境的改善及對感染環境有所防護等措施。
QuantiFERONÒ-TB Gold In-Tube (QFTIT) with the Mycobacterium tuberculosis specific-proteins ESAT-6, CFP-10 and TB7.7 to stimulate T cells and release interferon-gamma (IFN-γ) from the blood, to assess the possibility of TB infection. Blood samples were taken from contacts in the public health department and detected by enzyme-linked immunoassay (ELISA) for latent tuberculosis infection. Results: Total 6,753 blood samples, including 1,165 pieces of Department of Health, Taipei City Government, 5,325 pieces of Department of Health, New Taipei City Government, 255 pieces of Taipei City Hospital, and 8 cases of high-risk medical technologists. Department of Health, Taipei City Government of positive rate was 10.47% (122/1,165), Department of Health, New Taipei City Government of positive rate was 19.12% (1,018/5,325). Taipei City Hospital of positive rate was 17.65% (45/255). High-risk medical technologist (MT) of positive rate was 37.50% (3/8). Latent tuberculosis infection of positive rate was 17.59% (1,188/6,753).The data of contacts were show positive rate almost 18% and the high-risk for medical technologist (MT) was 38%. Contacts to TB patients will be early diagnosis and early treatment. The risk of TB infection, exposure to high-risk of medical groups should more attention to the improvement of the working and protection of the environment and other measures of infection.
