幾種益生菌具有改善膀胱過動症候群的作用

Beneficial Effects of Several Probiotics on Overactive Bladder Syndrome

膀胱過動症候群(Overactive bladder syndrome,OAB)表現為突然、不受控制的排尿。目前治療OAB的方法包括使用抗乙醯膽鹼素(anticholinergics)、鈣離子拮抗劑(calcium antagonists)阻斷部分神經系統傳遞。由於這些藥物可能會導致便秘、口乾舌燥和視力模糊,因此,吾等探討以益生菌作為替代療法的可能性。為了測試這種可能性,吾等使用10周齡Wistar大鼠經7天適應期後,每日給每隻大鼠管餵給予0.9%生理食鹽水(對照組,N=8)或懸浮在0.9%生理鹽水中的各種乾益生菌粉,劑量為10.3 mg/kg(N=8),連續30天。第31天,對這些大鼠進行麻醉,以手術為每只大鼠插入導尿管,將膀胱連接到壓力監測系統,以記錄膀胱壓力(bladder pressure)、膀胱容量(bladder capacity)、排尿頻率(urination frequency, times/min)。結果顯示,短乳桿菌(Levilactobacillus brevis)GKJOY與發酵乳桿菌(Lactobacillus fermentum)GKF3具快速調節膀胱壓力的效果,具緩解急尿症狀的潛力。菌株GKJOY與乳酸片球菌(Pediococcus acidilactici)菌株GKA4則有維持膀胱穩定性的效果,減少漏尿的可能。另外,菌株GKJOY和菌株GKF3具降低排尿頻率的趨勢。補充菌株GKJOY可顯著增加Wistar公鼠的膀胱容量及排尿延遲指數。上述研究結果推測這些益生菌產品可能藉由舒張膀胱逼尿肌(detrusor muscle)進而增加膀胱容量,從而減緩頻尿、漏尿之症狀,並有可能發展成為治療OAB的替代療法。

Overactive bladder syndrome (OAB) is manifested by a sudden, uncontrolled urination. Current OAB treatments involve the use of anti-cholinergics and calcium antagonists to partially block neural transmission. As these medications may result in constipation, dry mouth, and blurred vision, we explored the possibility of using probiotics as an alternative treatment. To test this possibility, 10-week-old Wistar rats were each given 0.9% saline (control group, N=8) or various dried probiotics suspended in 0.9% saline at 10.3 mg/kg (N=8) daily by gavage for 30 days after 7 days of adaptation. On Day 31, these rats were anesthetized, and each was surgically inserted a catheter to connect the bladder to a pressure monitoring system to record bladder pressure, bladder capacity, and urination frequency. Results showed that the products of Levilactobacillus brevis GKJOY and Lactobacillus fermentum GKF3 had a rapid effect on bladder pressure, demonstrating a potential to alleviate urgency symptoms. The products of GKJOY and Pediococcus acidilactici GKA4 showed the ability to maintain bladder stability, thus reducing the likelihood of urinary leakage. In addition, the products of both GKJOY and GKF3 were found to reduce urination frequency. These findings suggest that these probiotic products may increase bladder capacity by relaxing the detrusor muscle, leading to decreased urination frequency and leakage and that they can potentially be developed to become an alternative treatment for OAB.
