背景與目的:超音波導引下實行中心靜脈導管置放可以縮短置放時間,提高了成功率,以及降低併發症發生率。擬真教學已經越來越多地用於超音波導引下實行中心靜脈導管置放訓練,它允許年輕的醫師可以刻意練習以達到技術的純熟且不影響患者的安全。本篇文章將就如何實際執行中心靜脈導管置放的擬真教學,本院實行的現況和回顧文獻的佐證。研究方法:擬真教學包括4個部分,分別為觀賞教學錄影帶,利用powerpoint檔案將超音波導引下實行中心靜脈導管置放的重點再次強調並做超音波儀器介紹,接著將學員分為兩組,一組先接受超音波導引下內頸靜脈的解剖位置的訓練,另一組則使用中心靜脈導管置放模擬模型進行實際演練,接著兩組交換教學。研究結果:共85位住院醫師完訓。課程評值顯示,在上課時間足夠、模擬實作增加信心和影片可增進了解等4項回答「非常同意」及「同意」達100%;覺得對臨床有幫助及可降低併發症達98%;整體課程滿意度為93.7%。結論:我們希望透過推廣超音波導引協助中心靜脈導管置入,期望提升管路置放安全。我們將再追蹤超音波使用率,希望讓醫師在執行中心靜脈導管置入時,能有效利用超音波導引技術,增加置放成功率,減少併發症,提升病人安全。
Central venous catheter (CVC) insertion has risks including mechanical and infectious complications. Procedure complication rates are inversely related to the experience of the operator. Methods: We combine team work, video-training and simulation for echo-guided CVC and design a training program for CVC placement. Results: Total 85 residents were enrolled into this study. After CVC workshop, confidence to use echoguided CVC was 100%. Agreement of using simulation to improved echo-guided CVC was 98%. Conclusions: Simulation-based residents teaching program improves echo-guided central venous catheter insertion.
