測量肩斜角度與X 光入射角度對提升肩棘上肌投射照影像品質之探討

摘要

在診斷肩峰下夾擊(shoulder impingement)時，一張影像品質良好的肩棘上肌投射照(supraspinatus outlet view)，是非常重要的。但要在不同型態的肩膀、肩斜裡，照出一張良好的肩棘上肌投射照，X光從tube的射出，即Central Ray(CR)入射角度，要使用多少度的傾斜是一個關鍵。從2010年9月迄2011年5月，我們蒐集了200張肩棘上肌投射照，其中大部分以肩斜角度在10度至20度之間照攝，發現CR角度以15度向尾射入，配合好的影像條件，可得到最佳影像，作為臨床醫師診斷依據。

Shoulder outlet view for supraspinatus is crucial on diagnosing shoulder impingement. An appropriate outlet film conducive for image diagnosis is mandatory. We performed and analyzed 200 outlet films taken from Sep. 2010 to May 2011 and concluded that a shoulder inclination of 15 degree CR projecting a good antero-oblique scapular film (certain criteria also mentioned below), could make a shoulder outlet view good enough sufficient for more accurate diagnosis.
