The Treatment Effect of Sialendoscopy on Obstructive Sialadenitis Without Sialoliothiasis

Background: To demonstrate the effect on self-reported symptom severity change after diagnostic sialendoscopy for non-lithiasis obstructive sialoadenitis. Also, we intend to document how symptom severity change over time and may provide clues for the disease pathogenesis. Methods: A retrospective chart review was conducted. From December 2017 to December 2018, 77 sialendoscopies were performed on 52 patients of obstructive sialoadenitis without sialolithiasis. Numeric rating scale (NRS-11) was recorded on pre-treatment as baseline, postoperative one week, one months, and three months. Operative findings such as stenosis/stricture, mucous plug formation (inflammation), ductal dilatation were recorded as factors affecting symptom severity. Results: Pre-treatment NRS-11 was 5.6 and it was significantly decreased to 1.9 after one week (p < 0.001). However, a rebound of symptom score to 3.3 was noted during three-month follow-up but remained significant difference from baseline. Patients with duration more than six months had a significant baseline score and those with ductal stenosis had an insignificant effect on the symptom severity. For patients with ductal stenosis, a higher proportion was found to receive multiple sialendoscopic procedures. Conclusions: Diagnostic sialendoscopy without intervention procedures, provide a treatment effect on symptom severity. For those who had primary symptoms longer than six months, a significantly higher symptom severity was found. Ductal stenosis was most likely to be the primary cause, and it was also related to multiple sialendoscopic procedures.
