急性腸繫膜動脈血栓溶栓－案例報告
Thrombolysis of Acute Mesenteric Arterial Embolism- Case Report
摘要
急性腸繫膜動脈栓塞〈acute mesenteric artery embolism, AMAE〉是一種危及生命的血管性急症。該病起病急，發展迅速，而且其臨床表現缺乏特異性，往往造成診斷的困難，也常導致醫師的誤診，因而延誤診治，死亡率高達50% ~ 90%。本次報告為65 歲男性病患，因為腹痛來急診。主治醫師懷疑可能是急性腸繫膜動脈栓塞，在電腦斷層的影像發現上腸繫膜動脈完全沒有顯影。安排病患做血管攝影溶血栓，將導管放在血栓中並且固定，連接所配好的血栓溶素注射劑注入到血栓中。溶栓24 小時之後，再讓病患做血管攝影追蹤血栓情況，發現其上腸繫膜動脈血栓情況改善。之後沒有任何病發症，則讓病患出院。

Acute mesenteric arterial embolism (AMAE) is an acute life-threatening vascular disease which is difficult to diagnose due to its acute onset, fast development, and lack of specific clinical manifestations. It is often misdiagnosed by physicians resulting in delayed treatment, with mortality rate as high as 50-90%. The present case is a 65-year old male who came to the clinic complaining of abdominal pain. The attending physician suspected that the patient might suffer from AMAE and ordered computed tomography angiography (CTA). The results indicated that the patient's superior mesentery artery failed to show any images. The patient was then injected with thrombolytic agents through a catheter inserted and fixed at the spot of the embolus. After 24 hours of thrombolysis, the patient underwent another CTA to check the status of the embolus. It was found that his superior mesenteric arterial embolism improved and showed no complications. The patient was then discharged from the clinic.
