大腦靜脈竇栓塞(cerebral venous sinus thrombosis)會造成皮質水腫(Cortical edema)。大腦靜脈竇栓塞初期症狀表現與動脈栓塞所造成的缺血性中風相似,因此容易被誤診。本次研究主要是利用電腦斷層(computed tomography, CT)和磁振造影(magnetic resonance imaging ,MRI)及時和準確性地診斷大腦靜脈竇栓塞造成的皮質水腫。本文所列的病例為一位52歲男性,因為頭痛伴隨癲癇至醫院急診尋求協助,初步診斷疑似腦中風,經無顯影劑的電腦斷層檢查,而像上沒有發現明顯的出血狀況,只有在上矢狀竇處有發現高密度損傷的影像表徵。進一步在做磁振造影的檢查,結果證實其為靜脈栓塞所造成的水腫,而非動脈性的栓塞。
Cerebral venous thrombosis, often misdiagnosed as stroke due to similar initial symptoms and signs, can cause cortical edema different from that induced by arterial embolism. Initial symptoms of cerebral venosinous thrombosis are similar to those of ischemic stroke induced by arterial embolism and misdiagnosis is not uncommon. The aim of the study is to immediately and accurately diagnose cortical edema caused by cerebral venosinous thrombosis by using CT and MRI. The case presented here is a 52 years old male who visited emergency department (ER) because of headache followed by seizure. The initial impression was stroke and non-contrast enhanced CT was performed. CT image showed no evidence of obvious brain hemorrhage but high intensity inverse triangle lesion over superior sagittal sinus. Further MRI proved the lesion was edema cause by venous thrombosis instead of arterial embolism.
