胸部前後位前凸X光攝影之最佳化探討

摘要


放射醫學影像品質影響臨床醫師的診斷,因此在任何攸關影響影像品質之問題都應盡可能的去改善。同時在臨床操作上也因受檢者變動因素太多,放射師在操作儀器時需視臨床狀況而應變,例如病患需執行胸部前後位前凸攝影(Chest AP lordotic view)時,受檢者身體需站立傾斜或搭配管球向頭端打角度之方式執行,但教科書上並未明確指出站立傾斜角度為多少。研究方法是應用人體骨模型進行不同站立傾斜角度(胸骨柄與水平面夾角)之方式進行照射,另搭配不同管球角度進行chest lordotic攝影並評估影像品質。藉此研究找出chest lordotic攝影時受檢者直立傾斜最佳角度,以科學方式取代舊有目視方式提升影像品質,結果呈現投影角度在45度為較佳的方式,即是以直立照射時角度為45度,考量安全性時可以站立角度60度搭配管球向頭端打15度之方式為較佳條件。此研究結果能提供放射師在臨床執行chest lordotic攝影時的參考。

Quality of medical images can influence clinicians' diagnosis; therefore, any problems about imaging quality should be solved. Meanwhile, there are so many variable factors about patients in clinical practice that medical radiation technologists should make appropriate adjustment in taking medical images, such as when performing AP lordotic chest radiography, there is no definite instructions about the leaning angle of patients or the cephalad angle of the x ray tube. Our study method is to take AP lordotic chest radiographs in different leaning angles of patients (the angle between the sternal manubrium and the horizontal line) by using a human skeleton phantom, and to evaluate imaging quality. We try to define the best leaning angle of patients and to improve imaging quality of AP lordotic chest radiography by using a scientific method rather than just visual inspection. With different leaning angles of patients combined with different cephalad angles of the X ray tube in taking AP lordortic chest radiographs, we find the best leaning angle of patients is 45°. But when taking the patient safety into consideration, we recommend a 60° leaning of patients combined with a 15° cephalad of the X ray tube. We hope our study can provide medical radiation technologists a useful reference in performing AP lordotic chest radiography.
