The Benefit of Liver MRI with Free Breathing GRASP-VIBE Sequence

摘要


對於肝臟磁振造影而言,動態對比增強影像(Dynamic-contrast enhanced MR imaging, DCE-MRI)的三時相影像(動脈相、靜脈相和延遲相)佔有很大的重要性。要得到良好的影像品質,必須要病人能充份地配合吸閉氣的動作,倘若遇到重症患者、重聽或者意識不清的病人,往往會因為配合度不高而喪失影像的準確度。GRASP-VIBE序列採用特殊的採樣技術,可以在讓患者在腹部自由呼吸的狀況下進行上腹部動態對比增強掃描連續成像,而非傳統要求患者閉氣後在特定時間點進行掃描取像,然後再從連續取得的數據庫中利用疊代法演算,取得所需要的三時相影像。此種成像方法大幅度地減低了呼吸運動所造成的假影,同時降低動態對比增強影像的失敗率,對於臨床診斷上而言,將有機會幫助醫師提供更精準、更詳盡的判讀報告。

For liver MRI, dynamic contrast enhanced sequence (arterial phase, portal venous phase and delayed phase) is always important for clinical diagnosis. Conventional dynamic-contrast liver MRI sometimes provides poor image quality because of breath-hold issue. It offers fast, three-dimensional imaging and depends on breath-hold process which is about twelve to fifteen seconds.. But for those who can't cooperate with breath-hold process, image quality would be questionable in that situation. Free-breathing GRASP-VIBE sequence is an alternative choice at this time. Without breath-hold procedure, uncooperated or even unclear patients can still generate useful images for diagnosis. When we have patients like child, unconscious patients or even patients with endotracheal tubes, it still can obtain better image quality. The utilization of GRASP-VIBE sequence should be considered to improve diagnosis of liver MRI examination.
