Messenger RNA(mRNA)新冠肺炎(A coronavirus disease 2019, COVID 19)疫苗已在全球廣泛使用,以防止嚴重急性呼吸系統綜合症冠狀病毒2(Severe acute respiratory syndrome coronavirus 2, SARS-CoV-2)感染的進一步傳播。早期研究和監測數據表示,除了非常罕見的過敏反應外,這些疫苗與其他重大不良事件並未證實有明確之因果關係,其它反應的監測則仍在繼續進行當中。目前獲准在我國緊急使用於青少年的新冠肺炎疫苗為輝瑞BNT(Pfizer-BioNTech)。本案例為14歲男性無感染過新冠肺炎,於打完第二劑mRNA新冠肺炎疫苗二日後,感覺胸悶及咳嗽但沒有發燒,至醫院就醫檢查,結果發現血液中的心肌旋轉蛋白(Troponin I)值偏高,一般心臟衰竭、肺栓塞、心肌炎、心肌挫傷與腎功能衰竭這些疾病皆可能導致Troponin I值升高,因此本案疑似具有急性心臟相關疾病。住院治療期間安排心臟磁振造影(MRI)發現左心室下壁有明顯的高訊號及钆吸收,影像特性為心肌炎可能性較高。
Messenger RNA (mRNA) coronavirus disease 2019 (COVID-19) vaccines have gained widespread use across the globe to prevent further spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Early studies and surveillance data suggest these vaccines are associated with no significant adverse events other than very rare anaphylaxis. Surveillance for other reactions continues. The COVID-19 vaccine currently approved for emergency use in adolescents in my country is Pfizer-BioNTech (BNT). In this case, a 14-year-old man who has never been infected with COVID-19. After receiving the second dose of mRNA vaccine, two days later, he felt chest tightness and cough but no fever is detected. He went to our hospital for help and the results of blood test showed elevated serum Troponin I, and acute heart-related diseases were suspected. Cardiac magnetic resonance imaging (MRI) was arranged during hospitalization, regional high T2 signal and gadolinium absorption in the inferior wall of the left ventricle is detected. The imaging characteristics were suggesting myocarditis.
