以立體導引技術輔助乳房攝影細針定位術針對較深且較薄乳房病灶之案例探討

Using Tomography-Guided Needle Localization for Deeper and Thinner Breast Lesion-case Report

摘要


乳房攝影主要用於偵測乳房的微鈣化點群,而微鈣化點群可能是早期乳癌的徵兆,有85%~90%的原位癌(0期乳癌)會以微鈣化的型態表現,針對無法觸診摸到及超音波無法偵測到的微鈣化點群,可因此藉由乳房攝影檢查找出這些可能為乳癌的微鈣化點群,藉此達到早期診斷早期治療的目的。當乳房組織有不正常且密集的微鈣化點群經由臨床判讀評估的診斷為性質可疑,有必要進一步採檢的組織,則需藉由乳房攝影定位做病理切片確定診斷。本文主要是以「乳房攝影立體導引細針定位術(Tomography-Guided Needle localization)」,針對壓迫後乳房較薄、軟且微鈣化點群靠近胸壁後下方靠中間的位置,以乳房攝影立體導引細針定位術取代乳房攝影導引細針定位術,達到最適化的定位後,協助手術取樣切片的進行。

Mammography is mainly used to detect microcalcifications in the breast. Cluster of microcalcifications may be represented as breast abnormality because 85% to 90% of carcinoma in situ (stage 0 breast cancer) will manifest as microcalcifications. Mammography can detect these microcalcifications and impalpable breast lesions that cannot be detected by ultrasound, so as to achieve the purpose of early diagnosis and early treatment. When there are abnormal and dense microcalcifications within the breast tissue and cannot reasonably be assumed to be benign, further evaluation is required. Thus tissue sampling by mammography localization is recommended. This article is mainly based on "Tomography-Guided Needle Localization", aiming at the thinned, soft breast tissue and microcalcifications located close to posteroinferior medial chest wall after compression. Tomography-Guided Needle Localization can substitute Mammography-Guided Needle Localization to obtain the optimal positioning and accurate localization of the lesion for tissue sampling.
