根尖攝影是診斷多數牙齒病理極為依賴的工具,尤其是牙髓治療更是如此。然而,經驗不足的放射線技師或PGY牙醫師常因為X光片子擺放不正確等技術問題,造成病人需要重拍口內X光片。本研究設計一個定位片,與數位X光片包裹後使用,可輔助操作者容易精準對準牙位獲得高品質影像,避免削減牙根尖影像並且保護數位X光片免除刮傷。此新方式從2019年高雄長庚紀念醫院牙科被執行。結果顯示,在根管治療期間使用定位片可獲得更精準的根尖影像。此外,數位X光片不能進高壓鍋滅菌,須將數位X光片封包入片套中防止感染。根據高雄長庚紀念醫院於2019年調查,普通醫療機構每年購買6萬片套,約新台幣36萬元。據統計,高雄長庚紀念醫院牙科購買了6萬個自製片套,僅花費1.8萬新台幣。每個自製片套成本約0.3元新台幣,約為商業片套的20分之1。因此,自製片套和定位片使用在牙醫業可以節省許多經費,並擁有高品質影像。
Periapical radiographs are extremely important in diagnosing the majority of pathologic conditions, and also indispensable in conducting endodontic treatment. However, the new radiographers or PGY dentists may often utilize improper film placement. We designed a positioning marker of digital film to achieve accurate dental position when operators use it which can help capture high quality images and avoid cutting tooth root and protect the digital film from scratching. This new method was performed since 2019 in the Department of Dentistry, Chang Gung Memorial Hospital Kaohsiung Center. The result revealed that taking radiographs with positioning marker film during the root canal treatment can be more accurate to capture images on the periapical film. In addition, the digital x-ray film cannot be autoclaved, must be put into barrier envelopes to prevent the spread of infection. According to the survey of Department of Dentistry, Kaohsiung Chang Gung Memorial Hospital and Chang Gung University College of Medicine (CGMH) in 2019, general medical institutions purchased 60,000 barrier envelopes in one year which costs about 360,000 NT dollars. According to records, CGMH has purchased 600,000 self-made barrier envelopes that only cost 18,000 NT dollars. A self-made barrier envelope costs 0.3 NT dollars which is about one-twentieth of the commercial barrier envelope. In conclusion, the self-made barrier envelopes and positioning marker film in dentistry can produce high quality images while lowering costs.
