營養職類可信賴專業活動之重症單位營養照護—執行說明暨經驗分享

Entrustable Professional Activity (EPA) for Nutritional Care in Critical Care Units-Formulating Implementation Instructions and Experience Sharing

摘要


以勝任能力為導向的醫學教育(competency-based medical education, CBME)已成為現今國內外醫學教育的主流發展模式,為落實與推展CBME,以可信賴專業活動(entrustable professional activities, EPAs)教學模式把核心能力架構融入實際醫療任務並落實在臨床教學評估,從而培養出具有勝任能力醫學專業人才,使其能應對日益複雜和多變的醫療環境。在台灣,營養師職類EPAs發展也逐漸展開,由中華民國營養師公會全國聯合會於2022年8月1日公告台灣營養師可信賴專業活動(EPAs)第一版。本專案以辦理工作坊的模式,邀請來自不同層級醫院的營養職類臨床指導教師參與,通過小組討論方式進行EPA3-重症單位營養照護之任務描述、先備知識/態度/技能及評估進展更細部及具體執行經驗的分享及討論,討論過程結合各醫院之教學經驗並統整執行具體作為,更進一步進行執行說明之撰寫。此報告可作為未來重症營養照護教學導入EPAs時之參考。

Competency-based medical education (CBME) has become the major trend of development for medical education worldwide. To promote CBME, the entrustable professional activities (EPAs) teaching model has been introduced, integrates core competencies into practical medical tasks. Introduction of EPAs generally aims to cultivate medical professionals with the necessary competencies to handle increasingly complex and variable healthcare environments. In Taiwan, the development of EPAs for dietitians has also been gradually advancing. The Taiwan Dietitian Association (TWDA) established the first edition of "EPAs for entry-level dietitians in teaching hospitals in Taiwan (established in 2022)" on August 1, 2022. This project describes process of holding workshops that involve dietitians as clinical instructors from different levels of hospitals. Through group discussions in the workshops, participants discussed and formulated detailed description of task descriptions, prerequisite knowledge/attitudes/skills, and specific assessment methods needed for EPA3 (nutrition care in critical care units). The process involves integrated teaching experiences from various hospitals and consolidating suggestion and opinions. This report can serve as a reference for future implementing of EPAs into critical care nutrition education.
