營養職類可信賴專業活動之疾病飲食營養團體衛教—執行說明暨經驗分享

摘要


可信賴專業活動(entrustable professional activities, EPAs)是以任務為導向,將完成任務所需的多項核心能力整合,應用於醫療職場教學訓練的評核方式。這些專業任務以信賴等級作為評估能力進展的依據,提供臨床工作授權之參考。中華民國營養師公會全國聯合會在2022年8月1日正式公布第一版「2022年台灣營養師可信賴的專業活動(EPAs)」,以教學醫院二年期臨床醫事人員培訓計畫之營養師,或是無經驗的新進營養師為訓練和評估對象。疾病飲食營養團體衛教是醫院營養師常用的衛教活動,目的是讓病人或照顧者瞭解飲食調整對疾病控制的重要性,藉此強化病人飲食控制的識能與技能,提升自我疾病管理能力與生活品質。本專案目的是針對公告版EPA6-疾病飲食營養團體衛教之任務描述、先備知識/技能/態度及評估進展的實際作法,透過舉辦工作坊,由教學醫院的資深臨床教師共同討論與分享導入經驗的方式進行,並融合各家醫院對新進營養師教學訓練經驗進行意見整合及報告撰寫,以作為提供教學醫院營養部門或營養職類導入EPA6之具體且實用的參考。

Entrustable professional activities (EPAs) are task-oriented assessment method that integrates multiple core competencies required to complete tasks and applies them to medical workplace training. The trust level is used as the basis for competency progress and references for clinical work authorization. The first edition of the "EPAs for entry-level dietitians in teaching hospitals in Taiwan (established in 2022)" was officially announced on August 1, 2022, by Taiwan Dietitians Association (TWDA). The framework primary targets entry-level dietitians or those receiving post-graduate clinical training programs by Ministry of Health and Welfare (MOHW) in Taiwan. Group education for disease-specific nutrition is a health education activity commonly used by nutritionists worked in hospital. The general purpose is to let patients or care givers understand the importance of dietary modification in disease control, hence strengthen patients' nutrition literacy and skills in dietary control, and improve self-disease management capabilities and life quality. This project focuses on elucidating task descriptions, training to acquire prerequisite knowledge/skills/ attitudes, and methods for evaluating progress of performance with EPA 6 (group education for disease-specific nutrition) by holding workshop. Senior clinical instructors from eleven hospitals participated. Through consensus-building, the project integrates the experiences of different hospitals regarding the training of entry-level dietitians. This guide is expected to provide valuable references for the implementation of EPA6 for Taiwan's teaching hospitals.
