提升護理師對非侵襲性正壓呼吸器操作之能力
A Project to Enhance Nurses' Ability in Setting-up Noninvasive Positive Pressure Ventilation
摘要
背景：非侵襲性正壓呼吸器（noninvasive positive pressure ventilation, NIPPV）治療的成效在於病人與呼吸器的配合度，而護理師於其中扮演著重要角色，若護理師無足夠的照護知識及能力且對照護的自信心不足，無法提升病人於NIPPV治療期間的舒適、降低發生壓力性損傷或緩解焦慮，亦會影響NIPPV治療成效。本病房護理師之NIPPV操作能力完整率為72.4%，分析要因為：缺乏一致的NIPPV照護教學教材、缺乏NIPPV照護持續教育訓練、接觸使用NIPPV病人前沒有練習機會。目的：護理師之NIPPV操作完整率及認知正確率＞90%。解決方案：透過NIPPV教育訓練課程、製作NIPPV線上教學影片、執行技術考、運用照護口訣、訂定NIPPV檢核表、設計模擬情境提升護理師照護能力。結果：護理師之NIPPV操作能力完整率由72.4%提升為97.1%，對照護NIPPV病人之認知正確率由54.3%提升至92.4%且NIPPV相關壓力性損傷發生率由28.8%降至11.1%。結論：運用課程、情境演練及體驗等策略確實能提升護理師照護能力。

Background: The clinical outcome of noninvasive positive pressure ventilation (NIPPV) depends on cooperation between patients and nurses. Nurses, who play a vital role in NIPPV treatment, need to have sufficient nursing knowledge, ability, and confidence in managing patients' discomfort, relieving their anxiety, and reducing the occurrence of pressure injuries during NIPPV treatment The nurses' accuracy rate of setting-up NIPPV in this ward was 72.4%. The main reasons for this less-than-optimal accuracy rate included the lacks of a unified training program, continuous education about NIPPV, and opportunities in practice NIPPV's set-up before caring the patients. Purpose: This project aimed to enhance the nurses' accuracy rate to set-up NIPPV, to 90% and greater Methods: Strategies developed by the project included conducting training programs for caring for NIPPV patients, producing an online video on how to set-up NIPPV, arranging skills evaluation tests, designing an acronym to help nurses to learn, providing a checklist, and designing a situational simulation exercise for nurses to learn the NIPPV-related care Results: The accuracy rate increased from 72.4% to 97.1%. Nurses' knowledge on NIPPV's care increased from 54.3% to 92.4%, and the incidence of NIPPV-related pressure injuries decreased from 28.8% to 11.1%. Conclusion: Under this project, strategies such as education programs, simulation, and practice exercise indeed increased nurses' caring abilities and skills regarding NIPPV.
