Background. Killian-Jamieson diverticulum (KJD) is a rare esophageal diverticular disease. Because KJD presents as a heterogeneous mass lesion with multiple hyperechoic foci under thyroid ultrasonography (US), it may be misinterpreted as a malignant thyroid tumor. Then, further unnecessary invasive intervention may be performed. Methods. Here we reported a patient with multiple thyroid nodules. The thyroid US showed a mass lesion with size of 1.44 X 2.27 X 2.59 cm with multiple microcalcifications over left thyroid lobe. By the malignant appearance of US, a malignant thyroid tumor was considered. Results. After regular follow up of thyroid US in one year, the lesion posterior to the left thyroid was noted to be of varied size and echotexture. Further survey of neck computed tomography and esophagography confirmed the diagnosis of Killian-Jamieson diverticulum. Conclusion. KJD may be misdiagnosed as a thyroid nodule which contributed to the unnecessary invasive treatment, such as aspiration or surgical intervention. It's important to keep this disease in mind when interpreting the thyroid US.
