兒童COVID-19急性感染後症候群之臨床表現與中西醫觀點

摘要


Coronavirus disease 2019(COVID-19)急性感染後症候群,為確診或疑似感染COVID-19病毒3個月後,仍持續至少2個月的症狀,並對日常生活或身體活動功能造成影響。兒童急性感染後症候群盛行率為1.6~70.0%,其中最常見之症狀為疲勞、頭痛、關節肌肉疼痛、胸悶胸痛、呼吸困難、味覺或嗅覺異常等。其原因為急性感染後造成免疫系統失調、發炎損傷,或重症引發之後遺症,而出現呼吸、腸胃、神經、心血管系統的不適症狀,現代醫學採用症狀治療並定期追蹤。中醫觀點認為與餘邪未盡,失於調理,不知禁忌,宿疾復發有關。針對兒童COVID-19急性感染後症候群治療需明辨虛實、「伏火」或「新感」,並有無合併「食復」、「勞復」、「復感」,恢復期治則可採培土生金法、肺脾同治,兒童首重脾胃調養。若能整合中醫治療,有助於平衡陰陽,及早恢復健康生活。

Post-COVID conditions refer to symptoms that persist for at least 2 months after 3 months of confirmed or suspected SARS-CoV-2 infection and generally have an impact on daily activities. The prevalence of post-COVID conditions in children were reported about 1.6-70% and the most common symptoms are fatigue, headache, muscle ache, chest tightness, dyspnea, and taste or smell disorders. The possible mechanisms may be related with the immune system disorder, inflammatory damage, or severe sequelae caused by acute infection, that result in the respiratory, gastrointestinal, nervous, and cardiovascular systems. Guideline of post-COVID condition recommend treatment with symptom-relief medicine and keep regularly follow-up. From perspective of traditional Chinese medicine (TCM), post-COVID conditions may be related to the remaining evils, loss of care, lack of self-discipline in diet and daily activity, and recurrence of previous illness. According to TCM theory, the diagnosis of post-acute COVID-19 conditions in children require distinguishing between deficiency and excess, "lingering heat" or "new infection", and whether there is coexistence of "food recovery", "physical recovery", or "reinfection" and whether the disease recurs due to improper diet, labor, and re-infection. During the recovery period, treatment can adopt the method of reinforcing earth to generate metal and treating the lungs and spleen together. For children, it is particularly important to nourish the spleen. If Chinese medicine treatment can be integrated, it can help balance yin and yang and promote recovery of a healthy life.
