上頜竇提升術同期種植體植入初期穩定性分析

Initial stability analysis of implant with maxillary sinus augmentation and simultaneous implant placement

摘要


目的:分析上頜竇內、外提升術同期植入種植體初期穩定性的影響因素。方法:收集從2015年1月~2020年12月因單側或雙側上頜後牙區牙列缺損,需行上頜竇提升術並同期植入種植體的病例。對缺牙位點的竇嵴距及骨密度與上頜竇提升術同期置入種植體的初期穩定性進行回顧分析。結果:納入患者62人,共76顆種植體。其中上頜竇內提升術53例,上頜竇外提升術20例,內提升手術失敗轉行外提升手術3例。上頜竇內提升組竇嵴距為6.87 ± 2.3mm,骨密度為1340 ± 244。上頜竇外提升組竇嵴距為5.43 ± 1.2mm,骨密度為1224 ± 294。上頜竇內提升組中,竇嵴距及骨密度均與種植體初期穩定性之間無統計學差異(P >0.05);上頜竇外提升組中,竇嵴距及骨密度均與種植體初期穩定性之間無統計學差異(P >0.05)。結論:上頜竇提升術中,竇嵴距及骨密度不能完全作為判斷種植體初期穩定性的指標。

Objective: To analyze factors of the initial stability of implant with internal and external maxillary sinus augmentation and simultaneous implant placement. Methods: From January 2015 to December 2020, cases of unilateral or bilateral maxillary posterior dentition defects that required internal or external maxillary sinus augmentation and (simultaneous) implant placement were collected. A retrospective analysis of the bone density and sinus crest distance and the initial stability of the implant during the same period of maxillary sinus augmentation. Results: A total of 62 patients with 76 implants were included. 53 cases of maxillary sinus internal augmentation, 20 cases of maxillary sinus external augmentation, and 3 cases of failed internal augmentation were transferred to external augmentation. The sinus crest distance of the maxillary sinus internal augmentation group was 6.87±2.3mm, and the bone density was 1340±244. The sinus crest distance of the maxillary sinus elevation group was 5.43±1.2mm, and the bone density was 1224±294. In the maxillary sinus augmentation group and the maxillary sinus elevation group, there was no significant difference between the sinus ridge distance and bone density and the initial stability of the implant (P >0.05). Conclusion: The sinus crest distance and bone density cannot be used as indicators to judge the initial stability of the implant in maxillary sinus augmentation.
