目的:探討拔牙後立即種植及立即修復的可行性,以及具體的臨床操作技術要點。方法:採用Replace, Xive螺旋形種植系統,以及Endopore圓錐形種植系統,爲48例病員的69個單根牙行拔牙後立即種植,並對其中37個植體進行了立即修復。結果:共有64個植體成功完成骨整合,總成功率92.8%。使用Replace系統的28個植體27個成功完成骨整合,成功率96.5%,使用Xive系統的26個植體24個成功完成骨整合,成功率92.3%,使用Endopore系統的15個植體13個成功完成骨整合,成功率86.7%。37個進行立即修復的螺旋形植體中34個完成骨整合,成功率91.9%。結論:拔牙後立即種植及立即修復是一種切實可行的方法,螺旋型種植體更適宜於行立即種植及立即修復,獲得植體的初期穩定是取得拔牙後立即種植成功的關鍵,拔牙後立即種植及立即修復須選擇適當的病例方可進行。
Purpose: Patients with teeth extraction usually desire replace the losing teeth with dental implant as soon as possible .The purpose of this article is to found out the possibility of immediate implant placement after extraction and immediate implant loading. Materials and Methods: 48 cases treated with 69 dental implants immediately after teeth extraction. 28 implants are Replace Select tapered (Nobel Biocare); 26 implants are Xive (Friadent), and 15 implants are Endopore (Innova). 37 screw implants had been loaded with provisional crown immediately after implant placement. Results: 4 (92.8%)of the 69 implants had osseointegrated after 2.5 months in the mandible and 4.5 months in the maxilla.34 (91.9%)of the 37 immediate loading crowns successfully survived until final restoration. More then ninety percent of the patients were fully satisfied with both aesthetic and functional outcome at the time of prosthetic insertion. Conclusion: Immediate implant placement and immediate implant loading is a very useful resolution for teeth missing. Screw shape implants are more suitable for immediate implant placement and immediate implant loading. The primary stability of the fixture is the most important factor for implants survival, and not all the cases are suitable for immediate implant placement after extraction.
