乳腺癌前哨淋巴結活檢的臨床應用價值
The clinical application value of sentinel lymph node biopsy in breast cancer
摘要
目的：探討乳腺癌前哨淋巴結活檢（sentinel lymph node biopsy, SLNB）的方法和臨床應用價值。方法：2015年8月～2016年8月本科共收治41例乳腺癌患者，採取美蘭染色SLNB，進行術中冰凍診斷，並與術後SLN連續切片及免疫組化檢查作比較。結果：41例中，40例成功行SLNB，成功率97.5%，術中冰凍切片發現20例SLN癌轉移，20例陰性者術後行連續切片及常規免疫組化檢查發現2例微轉移。結論：美蘭染色SLNB成功率較高、安全、價廉，前哨淋巴結狀況對乳腺癌腋窩淋巴結轉移有重要預示價值，SLN連續切片聯合免疫組化檢查，可增加SLN微轉移的檢出率，減少假陰性率。

Objective. To explore the clinical value of sentinel lymph node biopsy in the treatment of breast cancer. Methods. 41 women with breast cancer admitted to our hospital from August 2015 to August 2016 had sentinel lymph nodes biopsy done with Guerbet injection. The results of intra-operative frozen sections of sentinel lymph nodes were compared with post-operation paraffin section plus immunoreaction. Results. 40 cases were successful in sentinel lymph node biopsy, the successful rate was 97.5%. Twenty cases of metastasis were found in intra-operation frozen sections of sentinel lymph nodes. In 20 negative cases in frozen sections, 2 cases of micrometastasis were found in post-operation paraffin sections plus immunohistological examination. Conclusion. Sentinel lymph node biopsy with Guerbet injection has high successful rate, good safety and low cost. The findings of sentinel lymph node have important predictive value in axillary lymph node metastases for breast cancer. Post-operation paraffin section and immunohistological examination of sentinel lymph node can increase the detection rate of micrometastasis, and reduce the false negative rate in intra-operation frozen sections.
