射頻消融治療老年心房顫動患者臨床分析
Clinical analysis of 30 cases with atrial fibrillation in elderly patients treated by radiofrequency catheter ablation
摘要
目的：分析導管射頻消融（RFCA）治療老年心房顫動患者的臨床效果。方法：於2010年1月～2015年6月在本院住院並隨訪接受導管射頻消融治療的30例老年房顫患者，男17例，女13例，年齡60～81（67.78±4.09）歲。其中陣發性房顫27例，持續性房顫3例。在三維標測系統指導下以環肺靜脈口消融術為核心聯合其他術式導管射頻消融治療，消融的主要終點為肺靜脈完全電隔離。術後連續跟蹤隨訪12個月以上，觀察手術的成功率、復發率、併發症、生活質量改善及臨床療效。結果：30例患者術後即刻成功率為100%。術後年消融成功率為63.3%。術後發生心包填塞1例，經心包穿刺置管術後緩解；術後穿刺部位血腫2例。結論：三維標測系統指導下導管射頻消融治療老年房顫是安全有效的。

Objective: To evaluate the efficacy and safety of radiofrequency catheter ablation (RFCA) on Atrial Fibrillation in elderly Patients. Methods: Retrospectively analyzed the clinical data of 30 cases Atrial Fibrillation in elderly Patients treated by RFCA. Results: The immediate success rate of the 30 patients was 100%. The one-year success rate after radiofrequency ablation was 63.3%. One case of postoperative pericardial tamponade occur, remission was achieved after pericardiocentesis. Two cases of puncture site hematoma occur. Conclusion: With the support of three-dimensional Rhythmia Mapping System, radiofrequency catheter ablation treatment for elderly patients with atrial fibrillation is safe and effective.
