提升糖尿病照護率及合併症防治檢查執行率之方案
Project for improving nursing care quality and preventing complications on diabetes
摘要
糖尿病（diabetes mellitus, DM）患者定期接受檢查及治療，能有效降低DM合併症發生。本專案旨在提升糖尿病照護率及合併症防治相關檢查執行率。現況分析原因有：患者未回診、對DM照護知能缺失、醫護人員不清楚收案流程、未落實轉介衛教及轉介流程不完善、未排檢及未明確告知檢查目的、檢查地點標示不明。解決方案有：改良DM監控系統設定、成立DM病友團體、改善醫令系統設定、宣導收案流程、落實轉介DM衛教服務、修訂轉介流程、新增衛教室指引標示。專案介入一年後，照護率由15.9%升至75.4%，檢查平均執行率由73.7%升至94.0%。顯示本專案推動有助於提升DM照護率及相關檢查執行率，進而提升DM患者之照護品質。

Diabetes mellitus (DM) patients receiving regular examinations and treatments can effectively reduce the occurrence of DM associated complications. This project aimed to increase the efficiency rate of diabetes care and the implementation rate of diabetic examinations related to the prevention and treatment of complications. Current analysis has identified relevant issues, including patients not returning for follow-up visits, lack of knowledge about DM care, unclear admission process for healthcare staff and providers, failure to implement referral education and ineffective referral procedure, failure to schedule examinations and clearly inform the purpose of the examination, and unclear labeling to the examination locations. The solutions include re-designing and improving the setting of the DM monitoring system, establishing DM patient groups, advancing monitoring and medical order information systems, promoting patient admission processes, implementing referral DM education services, amending referral procedures, and adding signage guidance to health education rooms. One year after the project execution, the average care implementation rate increased from 15.9% to 75.4%, while the examination rate in average increased from 73.7% to 94.0%, both reaching the threshold of the project. This project effectively improves the implementation rate of diabetes care and related examinations rate, thereby enhancing the quality of nursing care for patients with diabetes.
