自HPV疫苗上市以來,大都以女性作為HPV防治宣導的對象。然而隨著生物科技演進與國際趨勢影響,台灣的HPV防治推廣也由女性為主,逐漸擴及到男性,因此本研究希望了解HPV防治推廣過程中,針對同一疫苗但不同對象的推廣,隨著目標群眾的改變,HPV疫苗的資訊內容中呈現了何種性別差異,這樣性別化的疫苗推廣又會對疫苗使用者在理解資訊上帶來什麼樣的變化。 作為一質性研究,本研究以半結構式訪談為主要研究方法,參與式觀察與文本資料蒐集為輔。前者透過較深入的對話進而分析參與者的疫苗經驗及其對該疫苗的理解與詮釋;後者則是透過參與HPV防治相關實體或網路活動以及相關資料的收集,了解目前HPV防治趨勢與策略以補充訪談中無法得到的資訊。 研究結果發現男性疫苗使用者對於HPV疫苗的理解隨著政策與宣導內容的走向有所改變。不同於HPV與女性、子宮頸癌的連結,對男性而言則呈現與預防性病有關的趨勢,本研究指出這樣的疫苗論述也可能造成非預期的影響,強調HPV疫苗作為現今全球預防性傳播感染疾病的重要手段,政策推廣與衛教的規劃中應謹慎處理其中性別化與標籤化的現象,以建立健全的HPV疫苗資訊。
Since the launch of the HPV vaccines, they have mostly targeted women population. However, due to the biotechnological advancements in the recent years, international trends have begun to call for males to join the fight. HPV prevention and treatment has gradually expanded to men in Taiwan. The aim of this study is to understand how the discourses of HPV prevention promotion changes as the target population shifts from women to men, how gendered vaccine promotions affect both the users’ decision-making experiences and the public understanding of this vaccines. In this qualitative study, I conducted semi-structured interviews combined with participant observation and text analysis. I interviewed male users about their interpretations and vaccination experiences of HPV vaccine. I collected information and documents about current HPV prevention trends and the HPV vaccine promoting strategies by participating in relevant campaigns both online and offline. The study finds that male receivers’ understandings of the HPV vaccine changes along with different promotion strategies. In contrast with the linkage of HPV to cervical cancer in the women population, men use of HPV vaccine is linked to the prevention of sexual transmitted diseases. The gendered HPV vaccine promotion could also cause stigma toward male users and lead to an unexpected consequence in the disease prevention. As HPV vaccination being a crucial approach to prevent infectious diseases, policy makers should pay serious attention to gender issues that significantly shape people’s interpretation and decision making regarding whether to opt to the vaccination. For this regard, the research finding inform the policy in developing adequate communication regarding HPV vaccine promotion.
