含魚油之營養補充品對癌症病患營養狀況之改善

摘要


據統計,約50至75%的癌症患者有營養不良的問題,尤其手術後、化放療及惡病質的病人皆有營養不良的情況存在,適當的營養補充品給予可以減少感染、再住院率及死亡的發生。另外,臨床研究指出ω-3多元不飽和脂肪酸能有效改善發炎狀況,進而使營養素容易被吸收,因此本研究主要探討含魚油之營養補充品對癌症病患營養狀況之改善。 本研究分為完全補充組及部分補充組,分別納入32位及38位腫瘤患者,年齡分別為55-94歲及17-92歲。研究介入期為12週,分別於介入前、中、後 (第0週、8週、12週) 評估受試者之體位、血液生化值、營養狀況及攝食情況。結果顯示,完全補充組及部分補充組在第8週及第12週時,受試者之實際熱量攝取、蛋白質及碳水化合物的攝取量皆有顯著性提升,且身體質量指數、體重及皮下脂肪厚度亦有顯著增加。此外,血清白蛋白、前白蛋白及運鐵蛋白等營養指標皆有顯著增加;血清膽固醇、三酸甘油酯、肌酸酐、尿酸、鈣、鎂、鈉、鉀等生化數值則無顯著變化。 綜合上述結果,給予含魚油之營養補充品可顯著改善體位及營養狀況。

50%-75% of cancer patients have malnutrition problem. Malnutrition happened especially in patients undergoing surgery, chemotherapy, and radiation therapy. Nutritional supplement reduces the incidence of infection, re-hospitalization and mortality. Clinical researches indicate that omega-3 polyunsaturated fatty acids could improve the inflammation of cancer patients and promote the absorption of nutrients.This study was to understand the improvement on nutritional status of cause patients by the nutrition of supplement containing fish oil. This study was divided into full supplementation group and partial supplementation group of tumor patients. 32 subjects(55-94 years old)in full supplementation group and 38 subjects(17-92 years old)in partial supplementation group in this study. The experimental period was 12 weeks. To assess the biochemical analysis, physiological symptoms and dietary condition at the initial, 8 weeks and 12 weeks after intervention. Results indicated that full and partial supplementation showed very significant difference in anthropometric measurements, including body mass index (BMI), body weight and triceps skin-fold thickness (TSF). Dietary record showed that the daily energy intake, protein intake and carbohydrate intake were significantly increased after intervention. Serum albumin and pre-albumin were significantly increased. Serum triglyceride, cholesterol, creatinine, uric acid and electrolytes showed no difference. In conclusion, the intervention of fish oil containing nutritional supplement could greatly improve the anthropometric measurements and nutritional status.
