以人體試驗評估I纖珍珠對肥胖者不易形成體脂肪之功效:一項隨機、雙盲與安慰劑對照試驗

摘要


肥胖是現代人常見的健康問題,且其盛行率迅速增加,成為一個嚴峻的公共衛生 問題,對全球醫療和經濟帶來沉重負擔。生活及飲食型態的改變、多樣化的食物及高 熱量食物的隨手可得,都是導致肥胖的主要原因。肥胖也是許多代謝疾病的基礎,包 括代謝症候群、2型糖尿病和心血管疾病。除了調整生活作息、飲食控制、增強運動 習慣與使用藥物外,服用膳食補充劑是現在廣為群眾接受的方式之一。膳食纖維為常 見的食品配方,可有效改善身體質量指數 (body mass index, BMI)、體重和體脂肪。其中,菊苣纖維 (inulin) 廣泛存在自然界中,主要來自菊芋塊莖與菊苣根,是一種水溶性可發酵但不可消化的多醣體,在小腸中不會被消化與吸收,並且能在結腸中被發酵和利用,過去的研究顯示,菊苣纖維能幫助減肥及改善健康。本次試驗樣品為添加菊苣纖維的飲料食用珍珠 (I纖珍珠),本研究目的在於透過臨床人體試食進行一項隨機、雙盲、安慰劑對照試驗,評估I纖珍珠在改善肥胖人群的體重、體脂肪,以及血液脂質參數之功效。本實驗招募50位BMI ≥ 27 或腰圍 > 80cm,年齡20-50歲的女性,隨機分為兩組 (n = 25):(1) I纖珍珠組及 (2) 安慰劑組,參與研究之受試者每日服用2 包I纖珍珠 (100g/包) 或安慰劑,持續服用12週,並於介入前後進行身體組成數值測量、皮下脂肪測量、生物阻抗分析和血液檢測。實驗結果顯示,介入I纖珍珠12週後與第0週相比,在組內之體重、BMI、體脂肪重量、內臟脂肪程度及低密度脂蛋白膽固醇有顯著下降。據此,I纖珍珠能有效改善肥胖者身體組成及血液脂質參數,達到不易形成體脂肪之功效。

Obesity is a prevalent health problem among contemporary populations, and the rapidly increased prevalence of obesity has been a significant public health issue with considerable medical and economic costs worldwide. The major causes of obesity were the alteration of lifestyles and dietary composition, a wide variety of foods, and the easy availability of high calorie diets. Obesity is also associated with many metabolic disorders, including metabolic syndrome, type 2 diabetes, and cardiovascular disease. Dietary supplements are widely accepted as an alternative to lifestyle changes, dietary control, increased physical activity, and medication. Dietary fiber is a well-known food ingredient that can improve body mass index (BMI), body weight, and body fat. Inulin is widely present in nature and derived from the sunchoke tuber and chicory root that is a water-soluble, fermentable, and indigestible polysaccharide. Inulin is not digested or absorbed in the small intestine and is fermented and utilized in the colon. Previous studies have demonstrated that inulin is related to weight loss and improved health. The sample of the investigation is the inulin-enriched dietary bubble (also called I-fiber pearl), and the aim of the study is to evaluate the effects of I-fiber pearl on improving body weight, body fat, and serum lipid profiles of the obese subjects by a randomized, double-blind, and placebo-controlled trial. In this study, 50 female participants with BMI ≥ 27 or a waist circumference > 80 cm, aged between 20 and 50 years old, were randomly assigned to two groups (n = 25). The groups were (1) I-fiber pearl group and (2) Placebo group. The participants received 2 packs of I-fiber pearl (100g/pack) or placebo per day for 12 weeks. The health questionnaires, dietary questionnaires, anthropometric determination, subcutaneous fat measurement, bioimpedance analysis, and blood examination were explored after intervention. The results indicated that the body weight, body fat, visceral fat rating, and low-density lipoprotein-cholesterol (LDL-C) were significantly reduced after I-fiber pearl treatment. Accordingly, the consumption of I-fiber pearls significantly improves the body composition and serum lipid profiles in obese subjects as well as declines fat accumulation in the human body.
