以科學計量分析富含血小板纖維蛋白於牙醫學之研究

A scientometric analysis of platelet-rich fibrin in dentistry

摘要


富含血小板纖維蛋白 (platelet-rich fibrin簡稱PRF) 是第二代創新血小板濃縮技術,目前已廣泛應用於牙科臨床上,用來促進組織再生及傷口癒合。本研究目的是以科學計量分析 (scientometric analysis) PRF在牙科領域的全球研究趨勢及現況。從Web of Science (簡稱WoS) 資料庫中,以科學計量方法篩選至2022年6月30日期間所有發表的PRF文章,從實證醫學的觀點,探討PRF在牙科各領域的應用和研究趨勢及現況。以Excel 2022和SAS軟體進行統計分析,運用VOSviewer及Bibliometrix Biblioshiny R-package將關鍵詞彙整後,繪製成可視覺化的圖形。在WoS資料庫中,共篩選出562篇PRF的相關研究,自2006年出現第一篇以PRF為主題的研究至今,相關文獻發表數量逐年遞升,且具有統計學上的意義 (p<0.01)。文獻類型總數前三高依次為原創文章 (original article) 共326篇,約佔58.01%、綜述文章 (review) 共96篇,約佔17.08%,以及案例報告 (case reports) 共57篇,約佔10.14%。文獻設計總數前三高依次為臨床研究 (clinical study) 共113篇,約佔20.11%、隨機對照試驗 (randomized controlled trial) 共99篇,約佔17.62%,以及綜述與統合分析 (review and meta-analysis) 共96篇,約佔17.08%。文獻發表總數前三高的國家,依序為土耳其共91篇,約佔16.19%、印度共69篇,約佔12.28%,以及巴西共36篇,約佔6.41%。此外,PRF於牙科臨床應用最常見於口腔顎面外科共175篇,約佔31.14%、牙周再生共126篇,約佔22.42%,和植牙領域共105篇,約佔18.68%。關鍵詞結果顯示富含血小板纖維蛋白研究集中於骨再生和傷口癒合,而拔牙窩是富含血小板纖維蛋白最常被放置的位置,可見其目前應用趨勢。另一方面,富含血小板纖維蛋白可以減緩疼痛的能力,以及越來越多的富含血小板纖維蛋白衍生物的產生,顯示其仍有待被發掘的潛力。綜合研究結果顯示,透過科學計量分析發現,PRF的研究文獻數量逐年攀升,發表文章形式集中於臨床研究與隨機對照試驗,提供了實證醫學作為臨床應用之佐證。而透過不同的計數方法以及科學計量分析圖的呈現,可以為富含血小板纖維蛋白的發展有更客觀的結果呈現。

A second-generation platelet concentrate called platelet-rich fibrin (PRF), which has been extensively employed in dentistry to promote healing and regeneration, was first described by Choukroun et al. Nevertheless, a scientometric examination of PRF in dentistry is still rare. In order to set a foundation for future work on the popular or developing themes during the past 20 years, the purpose of the study was to investigate and provide a scientometric analysis of PRF usage and its dynamic developments in the dental application. On June 30, 2022, a literature search was conducted in the Web of Science Core Collection database to examine studies about PRF. All the bibliometric data was collected in Microsoft Excel, interpreted by SAS statistical software, and displayed by VOSviewer and Bibliometrix Biblioshiny R-package software. Following exclusion, 562 articles were found and reviewed. There was a steady publishing pattern (p<0.01) and a considerable uptick over the previous five years. Original articles (n=326, 58.01%), reviews (n=96, 17.08%), and case reports (n=57, 10.14%) made up the three primary publication genres. Clinical studies (n=113, 20.11%), randomized controlled trials (n=99, 17.62%), and reviews and meta-analyses (n=96, 17.08%) were the three main study types. Turkey (n=91, 16.19%), India (n=69, 12.28%), China (n=40, 7.12%), the United States of America (USA) (n=39, 6.94%), and Brazil (n=36, 6.41%) were the five main nations that contributed the most publications overall. PRF was frequently used in implant therapy (n=105, 18.68%), oral surgery (n=175, 31.14%), and periodontal regeneration (n=126, 22.42%). The result of keywords indicated that the study of PRF mainly focused on bone regeneration and wound healing and the extraction socket was where PRF placed the most frequently which shows its current application trend. On the other hand, the pain-relieving ability of PRF and the increasingly newly developed PRF derivatives, show their potential still to be discovered. Overall, the results indicated that the PRF studies elevated yearly. Particular attention was paid to clinical researches and randomized controlled trials in the study design, which provided the solid groundwork for evidence-based science in the dental field. In addition, accurate findings for the scientometric analysis of PRF can be presented by integrating several counting techniques and data visualization tools.
