支架披覆內皮細胞之血液流動動態應力研究

摘要


冠狀動脈心臟疾病是現今工業化國家社會的主要健康問題,亦是造成人類死亡的主要原因之一。可以造成血管壁增厚的原因很多,其中內皮細胞所分泌的內皮細胞相關舒張因素,及內皮細胞相關收縮因素會影響到血壓和血管結構的變化。本研究的目的是模擬與支架植入血管壁後,支架已貼附在血管壁內側,內皮細胞覆蓋支架時,依內皮細胞增殖不同程度而進行比較支架附近的血流動力學現象。於論文中我們使用有限元方法來分析和模擬。有限元模擬能更好地分析抽象的血液流動現象和流動特性,如流速分佈。模型的結構是參考市面上已上市之支架。在研究中,模擬可分為三個架構與兩個階段。架構分別為內皮細胞增殖的完全覆蓋支架和覆蓋的50%和10%的支架。第二階段則為比較不同速度之血流所產生的流動壓力場和流動現象,這些不同的情況是我們所關注的項目。在臨床上,內皮細胞增殖完全覆蓋支架是最好的情況。但是當血管內皮細胞增殖過多,就會造成再狹窄。反之,覆蓋率低的內皮細胞會導致血栓形成。

The purpose of the present study is to simulate the hemodynamic phenomena of the place near the vascular wall with stent implanted. After the stent have been pasted on the endothelial cells, the coverage of the stent by the proliferation of the endothelial cell has different degrees. We use the finite element method to analyze and simulate. The finite element simulation is better presented in the abstract of blood flow phenomena and flow characteristics such as distributions of the flow velocity. The model structure was reference commercially available to stent. In the present research, the simulations can be separated into three parts. The proliferation of endothelial cells completely covers the stent and covered 50% and 10% of the stent. The pressure fields and flow phenomena of these different situation are what we are concern. In the clinical, the proliferation of endothelial cells that completely covered the stent is the best condition that we hope. But when the endothelial cells proliferate too much, it will cause restenosis. Contrarily, the low cover rate of the endothelial cells will cause thrombosis. In the present preliminary simulation, there is a disturbed flow near the stent.
