慢性牙周炎患者之牙齦溝液Th1/Th2細胞激素表現之探討

摘要


細胞激素被認為在免疫調節方面扮演了一個重要的角色。而牙周病則可為免疫反應的結果,因此可用來探討細胞激素的病理潛在作用。本研究的目的是探討慢性牙周炎(chronic periodontitis)病人之牙齦溝液(GCF)中,細胞激素Th1/Th2之表現。我們將分析經在治療前與治療後,病人GCF內之細胞激素(分為Th1與Th2兩大類)與一些選定之牙周致病菌之關係。牙齦溝液及牙齦下牙菌斑取自12位慢性牙周炎患者與7位健康控制組,以流式細胞儀分析IFN-γ、TNF-α、IL-2、IL-4、IL-5、IL-10之濃度及總量;以免疫螢光染色法及暗視野顯微鏡計算B.forsythus, C. rectus, P. gingivalis及P. intermedia所佔之比例。結果顯示在慢性牙周炎患者牙齦溝液中,TNF-α之總量較健康組高,兩者之間達到統計學上的差異,而IL-2之總量則在邊緣狀態。在經過非手術性牙周治療之後,IL-5及IL-10之總量增加的部位亦達到統計學上的差異。本實驗顯示在慢性牙周炎的炎性過程之下,Th1類的反應是較顯著的,而Th2類的反應則是偏向組織癒合性之反應。

Cytokines are considered to play important roles in immunoregulation. Since periodontal diseases can be a consequence of the immune response, these diseases provide an important model for investigation of the pathological potential of human cellular infiltrates. To ascertain the cytokine profile in periodontitis lesion, our aim is to investigate cytokine Th1/Th2 profiles in GCF of adult periodontitis patients. We analyzed cytokine profiles(Th1/Th2) from adult periodontitis patients before and after periodontal nonsurgical treatment and the correlation of some periodontopathogens. GCF and subgingival plaque samples were collected from 12 patients with untreated chronic periodontitis and 7 healthy control subjects. IFN-γ, TNF-α, IL-2, IL-4, IL-5, IL-10 were analyzed by cytometric bead array. The profile of B.forsythus, C. rectus, P. gingivalis and P. intermedia were analyzed by immunofluorescence and darkfield microscopy. The results showed that higher TNF-α total amounts were found in patients than in healthy controls in GCF samples, and the IL-2 total amount were found at border line. After nonsurgical periodontal treatment, significantly increased sites in the total amount of IL-5 and IL-10 were found. In conclusion, our findings are in favor of the predominance of Th1 response in destruction, whereas the Th2 activities may be more relevant to healing in chronic periodontitis.
