下顎義齒頰棚區之可預測再現
Predictable Reproduction of the Buccal Shelf Area in Mandibular Dentures
摘要
下顎全口義齒（mandibular complete denture）的支持都會認為可以使用頰棚（buccal shelf）這區域。然而不幸的是，全口義齒經常未能完全的覆蓋住頰棚，這主要是因為在義齒製作時的印模階段，在頰棚區延伸不足所致。 這次研究的目的是來評估牙周探針的使用測量，對下顎全口義齒的頰棚區能有可預測再現性。而且確認一般牙科診所所製作的全口義齒在義齒頰棚區的寬度和口內頰棚區的寬度是否有所差異。 此研究是針對100位在牙科診所製作全口義齒（complete denture）的病人進行測量。測量內容包括病人口腔內的解剖構造和下顎全口義齒結構兩部分。病人口腔內解剖構造的測量範圍是頰棚區域的寬度。將頰棚分為三個次區域來測量，即頰棚的前端、後端和中間三部分。頰棚的前端即頰棚區在頰繫帶處的寬度；頰棚的後端是頰棚區在臼齒後墊前緣的寬度；頰棚的中間段則是指前端和後端兩部分之中間的頰棚區域的寬度。下顎全口義齒結構的部分則是測量病人下顎義齒的頰側凸緣（buccal flange）和受測病人口內測量處相對應位置的寬度，因此也分成前、中、後三部分。 測量結果發現頰棚區域寬度，在前端最窄平均為8.8毫米（mm）；往後逐漸變寬到中間寬度約11.1毫米（mm）；而後端則最寬，平均寬度有13.2毫米（mm）。發現不論在前、中、後三部分，下顎全口義齒相對應於頰棚處的寬度均比口內頰棚區域寬度來得少。 臨床醫師可以藉著使用牙周探針來測量頰棚的寬度，使得義齒在頰棚區能有可預測再現。而且確認一般牙科診所所製作的全口義齒在義齒頰棚區的寬度和口內頰棚區的寬度有顯著差異。

Purpose: The objectives of this study were to assess that a periodontal probe measurement would indicate a predictable reproduction of the buccal shelf areas in mandibular dentures and to identify that there are significant differences between the widths of the buccal shelf and the corresponding portions of mandibular dentures from dental clinics of general dentists. Materials and Methods: One hundred patients were measured in anterior (buccal frenum area), middle (center of anteroposterior width of buccal shelf) and posterior (anterior edge of retromolar pad) width of buccal shelf with a periodontal probe. This measurement was then made with that of the corresponding portion of the existing denture. Results: The mean width of the anterior buccal shelf is about 8.8 mm, the middle portion is about 11.1 mm, and the posterior portion is about 13.3 mm. At each portion, there are statistical significant differences in the width of the buccal shelf and that of the corresponding portion of the existing denture. Conclusion: The clinicians can use the periodontal probe to measure the width of the buccal shelf to indicate a predictable reproduction of the buccal shelf areas in mandibular dentures. There are significant differences between the widths of the buccal shelf and the corresponding portions of mandibular dentures from dental clinics of general dentists.
