本研究目的為以機械應力設備量測跟腱組織特性的可信度,並了解年齡與跟腱炎個別對跟腱組織特性的影響。 參與本研究計畫者共有50人,正常組為年輕族群年齡20~40歲之間者15人,中年族群為年齡40~60歲之間者15人,老年族群為年齡60~80歲者10人;跟腱炎組則有8人,年齡在20~60歲之間。 測試方法為請受測者放輕鬆在趴臥姿勢下將小腿露出並垂出治療床緣,讓踝關節呈現自然擺放姿勢,紀錄跟腱總長度後並進行等比例的區域劃分,將跟腱劃由上而下分為四個區域,測量位置為第三區中點,接著使用Myoton儀器位於做記號部位的中間點上進行測量,同時測量兩腳,每個位置測量3次取平均值。 結果顯示,同天測量的三個數值之間有高的組內相關係數,而在不同天之前、後測的內部一致性方面亦有良好的信度,在年齡組的部分,老年組的彈性明顯優於年輕組,而跟腱炎組在健、患側腳的分析結果以及與正常人組的比較顯示,患側腳的肌腱剛性比健側與正常人組低,而患側腳相較於健側與正常人組亦有較長的潛變與應力鬆弛時間,此將影響其對身體的重量剛性支撑能力。
The purposes of this study were to verify the repeatability of the Myoton PRO myotonometer measurements on viscoelastic properties of the Achilles tendon and investigate the effect of aging and disease (Achilles tendinitis) on the viscoelastic properties of the Achilles tendon. Forty normal participants were recruited into the present study and organized by age into study groups: younger (age from 20-40 y/o, n=15), middle-aged (age from 40-60 y/o, n=15), and older (age from 60-80 y/o, n=10). In addition, 8 participants with Achilles tendinitis were recruited into patient group (age from 20-60 y/o). To measure the viscoelastic properties of the Achilles tendon, participants were asked to prone comfortably and place their ankle out of the bed with a resting position. The length from the middle point of calcaneus tubercle to the connection point of calf muscle and calcaneus was record and then divided into 4 areas equally from up to down. In the present study, the testing position was set on the middle point of the third area from the up. For three age groups, each participant was measured three times on both legs at the same testing day and retested at 1 week later. For patient group, participants took the tests only once. The results show that there were high intraclass correlation coefficient on the within-day and between-day testing. While discussing the aging effect, it shows a significant difference on the elasticity parameter (D) between the elderly group and the younger group. While discussing Achilles tendonitis effect, the results show that the involved side leg has significantly lower tendon stiffness (S) than the uninvolved side. However, the creep ability (C) and stress relaxation time (R) on involved side leg was significantly higher than uninvolved. Besides, the Achilles tendinitis group shows similar results such as significantly lower muscle tone and stiffness, higher creep ability and stress relaxation time on involved side than age-matched normal group. This will affect their ability to support the body weight.
