背景:隨著我國生活水準提高,老人假牙的使用率已經是非常的普遍,高雄市政府自民國八十八年七月起,為提昇老人生活品質、促進身體健康,首創65歲以上老人公費裝置假牙計畫,自第1期開始至民國九十五年(第七期)已完成公費裝置假牙人數約25,000人。老人假牙的使用率已經相當的普遍,此時如何製作出一副實用性高的假牙在臨床上是相當令人關切的問題。 研究目的:1.探討義齒配帶情形與舌頭位置之關係。2.探討缺牙位置與舌頭休息位置之關係。3.依性別、年齡、及有牙、部份無牙之舌頭位置探討其差異。 研究方法:本研究已通過高雄醫學大學附設中和紀念醫院,人體試驗委員會之審查。在膺復牙科門診、安養機構、社區口腔健檢,針對45歲以上配戴部份義齒之病人及自然牙者,進行口腔檢查及問卷調查。口腔健康檢查由牙醫師執行。將收集的問卷資料進行編碼,並鍵入JMP5.12(p<0.05為有意義)統計套裝軟體中,在資料檢查與修正後,以JMP5.12統計套裝軟體進行統計結果分析。 結果:配帶局部義齒之病人,Wright舌頭位置分類為正常佔29.29%,退縮舌頭位置佔69.23%,達統計上顯著性差異(p<0.001)。自然牙者,Wright舌頭位置分類為正常佔70.71%,退縮舌頭位置佔30.77%,達統計上顯著性差異(p<0.001)。後側缺牙依Kennedy分類法分類KennedyⅠ佔(58.93%)。 結論與建議:1.臨床牙醫師應該加強口腔衛生教育及全口重建的觀念。2.後側缺局部無牙之病人(KennedyⅠ及KennedyⅡ之病例)隨著年齡增加,舌頭後縮的機會越大。3.可作為牙醫師製作後側無牙之局部假牙時之診斷參考,以增進假牙之穩定性及固位性。
Background: Following the enhancement of living standard in Taiwan, the population of its denture usage has been increasing dramatically. Since July of 1999, in order to promote better living quality and health for its local ageing population, Kaohsiung city government has established their first “Free Denture Plan” for the local residents whose age above 65. Within the first seven years, there are 25,000 people benefited from it. As the old person denture has become hugely popular universally, how to make a set of highly practical and usable denture is essential. Research: 1.To study the relationships between the denture and tongue position. 2. To study the relationships between partially Edentulous and tongue retraction position. 3. To investigate the tongue position followed by gender, age and teeth availability for the dental patient. Research method: This research has been examined and approved by the human body experiment committee of Chung Ho Memorial Hospital at Kaohsiung Medical University. Oral examination and survey has been carried out to those who are denture patients and natural teeth who are above 45 years old by qualified dentist from local dental hospitals, clinics and other organizations. All the surveys were analyzed and classified by JMP5.12 (p<0.05 was statistically significant). Result: Partial denture patient, Wright’s tongue position is classified as 29.29% for normal, 69.23% for tongue retraction position, p<0.001 (was statistically significant). Nature teeth, Wright’s tongue position is classified as 70.71% for normal, 30.77% for tongue retraction position, p<0.001(was statistically significant). Conclusion: 1.) The dental practitioner should increase the awareness of the oral hygiene education and the oral reconstruction. 2.) The partially Edentulous patient (Kennedy I and Kennedy II cases) will result with a higher possibility of tongue retraction by increasing age.3. )This can be the reference of stability and dentition for the denture for dental practitioner.
