應用HPLC-ELSD偵測社區民眾尿糖含量與簡易健康指標相關之研究

摘要


應用本實驗室已建立的HPLC-ELSD分析方法,對人體尿液中葡萄糖含量進行偵測。比較本方法分析之尿糖值,指端血糖值,及簡易健康指標 (BMI、腰圍、體脂肪率、內臟脂肪率) 間的關聯性。 樣本來源為高雄市十個社區及雲林縣一個社區的民眾,尿液檢品回收394份。男性109位,女性285位,平均年齡58.2歲。將民眾分為不自認為有糖尿病與自知有糖尿病,問卷上填寫為自知有糖尿病57位 (14.5%)。二組測得之尿糖值分別為:64.5 ± 277.3, 484.0 ± 750.5 mg/dL,指端血糖值分別為:109.4 ± 25.6及176.8 ± 66.1 mg/dL。尿糖值與指端血糖值在統計上的相關性顯著 (P < 0.01)。所有簡易健康指標與血糖值在統計上均有顯著相關性 (P < 0.05),BMI、腰圍、內臟脂肪率與尿糖值有顯著相關性 (P < 0.05),體脂肪率與尿糖值的相關性 (P value 0.695) 則未達到顯著意義。 本方法沒有指端血糖機扎針的侵犯性,又比現今尿糖試紙 (偵測極限50 mg/dL) 具更靈敏的偵測感度。未來可配合血糖檢驗應用在糖尿病患以監測其疾病控制情形,及早發現控制不佳便檢討改善其治療模式,降低糖尿病的惡化與併發症帶來的危害。並具有在社區中及早偵測出尿糖值30 ~ 96 mg/dL之可能潛在糖尿病危險群的能力。

The analysis of the application of HPLC-ELSD method to determinate the urine glucose concentration. The urine samples were collected from 10 communities of Kaohsiung and 1 community of Yunlin by community health examination service. Analyze the correlation between the urine glucose content and the fingertip blood glucose, the basic health indicators (body weight, body mass index, the waistline, body fat percentage, and visceral fat level). The urine sample size is 394, including 109 male and 285 female. Ranging in age from 15 ~ 86 years, the mean values of age is 58.2 years old. The subjects were separated to diabetes and general group, including 57 and 337 people. The analysis result showed that the mean values ±standard deviation of urine glucose concentration in diabetes and general groups were 484.0 ± 750.5, 64.5 ± 277.3 mg/dL. Among both groups, the results of the urine glucose concentration were statistically highly correlated with the fingertip blood glucose (diabetes group r = .670, p < 0.01; general group r = .609, p < 0.01). Using the HPLC-ELSD method to determinate the urine glucose concentration is non-invasive, and more sensitive than the urine glucose strip. It has the potential to apply to the disease monitor of diabetes patients’ control. And it has the ability to sieve out the possibly high risk group of diabetes in community residents. Provide the opportunity for early prevention the development of diabetes.
