社會工作者服務思覺失調症患者的經驗與反思

The Social Workers’ Experience and Reflections on Serving Patients with Schizophrenia

摘要


自從105年小燈泡的新聞事件發生之後,思覺失調症與隨機或無差別殺人開始連帶受到大眾的關注,於是誕生了「社會安全網」,也正因為我有這個機緣成為心理衛生社工,在服務了幾年之後,看到慢性思覺失調症患者幾乎都較為退化、退縮,與我們印象中的暴力行為,幾乎很難畫上等號。於是便想開始了解與自己一樣投身於服務思覺失調症的網絡成員中,是否也曾經出現與我相同的迷思,我們在服務時,是否曾經想過自己究竟有沒有對服務對象汙名化過?而我們看到了他們被汙名化的社會現象之後,除了提供衛教、關懷、解惑、陪伴之外,我們還能夠為他們做些甚麼? 於是,本研究尋找6位服務年資超過五年以上的社會工作者,經過訪談及分析後,其研究結果如下:(一)即使在服務的過程中,每位受訪者都有著不同程度的內心衝擊,或多或少也都有著挫敗或成功的經驗,但所有的受訪者都非常樂意繼續深耕於服務思覺失調症,也都十分理解擔任照顧角色的家屬辛勞與困境。(二) 在媒體過度渲染的報導之下,受訪者替個案求職大不易、在社區間工作遭誤解、看到機構內個案的交友權利之受限、夾在家屬與機構之間的為難、當自己成為家屬身分時,都能夠感受到隨處可見的汙名化,也在在地影響了社會工作者的專業服務。(三) 社會工作者反思後,開始重新思考暴力事件的框架、也了解社區服務的不足,而且我們可以對待個案像朋友一樣的相處方式幫助他們。(四)對於本研究提出的建議,則是社會工作者配合政府政策配合宣導,讓醫療與社工服務可以展現較明顯的效果、倡議家屬也需要被照顧、增進社工的服務多元化以貼近需求、布建社區資源……等,給予社會工作者參考之。

Since the news incident of Little Light Bulb in 2016, Schizophrenia and random or indiscriminate killings have begun to attract public attention, so the "Social Safety Net" was born. It is precisely because of this opportunity that I became a mental health social worker. After several years of service, I have seen that almost all patients with chronic Schizophrenia are regressed and withdrawn. It is almost difficult to equate them with the violent behavior in our impression. So I wanted to start to understand whether the network members who are devoted to serving people with Schizophrenia like me have had the same myths as me. When we are serving, have we ever thought about whether we have stigmatized the clients we serve? Pass? And after we see the social phenomenon of their being stigmatized, in addition to providing education, care, solving doubts, and companionship, what else can we do for them? Therefore, this study looked for 6 social workers who have served for more than five years. After interviews and analysis, the research results are as follows: (1) Even in the service process, each interviewee has different degrees of They all have more or less experiences of frustration or success, but all interviewees are very happy to continue to work on serving patients with Schizophrenia, and they all fully understand the hard work and plight of family members who take on the role of caregiver. (2)Under the over-exaggerated reports in the media, the interviewees found it difficult to find jobs for patients, were misunderstood when working in the community, saw that the rights of patients in the institution to make friends were restricted, were caught between family members and the institution, and faced difficulties when they became When people are family members, they can feel the stigma everywhere, which also affects the professional services of social workers on the ground. (3) After reflection, social workers began to rethink the framework of violent incidents and understood the shortcomings of community services. We can treat cases like friends and help them. (4) The suggestions put forward by this study are that social workers cooperate with government policies and publicity, so that medical and social work services can show more obvious effects, advocate that family members also need to be taken care of, and increase the diversification of social workers’ services to meet needs. , deploying community resources, etc., and providing them as a reference for social workers.
