我國長照產業人力資源發展策略之探討

摘要


台灣正面臨高齡化社會結構不斷攀升,失智與失能人口的增加,國人對於長期照顧服務的需求量與日俱增。現今社會與時代變遷,家庭人口數減少與性別平權觀念逐漸深植人心,小型家庭的人口型態對於高齡父母照顧,非常明顯的在人力上的侷限現象趨勢增加社會不斷進步的驅使下,人們生活的品質提升,面臨龐大的經濟壓力,促使家庭主婦的就業率不斷創新,家庭照顧的工作,也逐漸轉移至專業的居家服務員、照顧服務員,甚至充分依賴外籍的移工身上。機構住宿式長照服務是可以提供最全面的長期照顧;其主因是機構住宿式長照服務是直接將醫療等級的器材與設備列為每一床位的配備,並依照病患的照護需求進行安置,且24小時聘請專業的護理師進行評估與照護,必要時會有專科醫生進駐觀察,因此在設備、人力、醫療與可容納病人病況容許度上都是長期照顧上最高規格,許多醫院也與長期照顧機構相互支援報備,延伸照護資源此乃機構住宿式長照服務的特性。是全時照顧也就是24時的照護型態,因此一直是需要大量的人力資源的投入,長期照護產業面臨人力資源現狀一直處於長期量能不足與留任率低的嚴重與急迫性困境。 本研究採質性研究深度訪談方式進行蒐集文獻與資訊,針對長期照護產業人力失調-「機構住宿型照顧服務員構面」所面臨供需失衡、社會價值觀感侷限之框架、發展角色定位、人才培育之策略、體制及政策體系。訪談長期照護產業業者或具有十年以上長期照護經驗之高階主管,探討第一線照顧服務員實務需求量狀況,期許政府政策面就長照產業照顧人力發展規劃、替補人力法規應更具適切性更臻周延。積極重視人才多元化與實務技能的培育,跨專業團隊之產學合作,我國著重長期照護產業人力資源發展策略的正確方向,方能減緩長期照顧人力缺口與提升留任率。冀望照顧服務員人力能滿足我國長期照護需求,解決長期照護產業照顧人力需求的難題,提升長期照護的品質與重要性。

Taiwan is facing an aging social structure, and the number of people with dementia and disability is increasing. The demand for long-term care services is increasing day by day. In today's society and the changes of the times, the family population is decreasing and the concept of gender equality is gradually taking root in the hearts of the people. The demographic pattern of small families is very obvious for the care of elderly parents.The improvement of the quality of the housewives and the huge economic pressure have prompted the continuous innovation of the employment rate of housewives, and the work of family care has gradually shifted to professional home attendants, care waiters, and even fully reliant on foreign migrant workers. The institutional residential long-term care service can provide the most comprehensive long-term care; the main reason is that the institutional residential long-term care service directly lists medical-grade equipment and equipment as the equipment of each bed, and arranges them according to the care needs of the patients , and 24-hour professional nurses are hired for evaluation and care, and specialist doctors will be stationed to observe when necessary. Therefore, the equipment, manpower, medical treatment and the tolerance of the patient's condition are all the highest standards for long-term care. Many hospitals also cooperate with Long-term care institutions report mutual support and extend care resources. This is a feature of institutional residential long-term care services. It is a type of full-time care, that is, 24-hour care. The long-term care industry is facing the serious and urgent dilemma of long-term shortage of human resources and low retention rate. This research uses qualitative research in-depth interviews to collect literature and information, aiming at the imbalance of supply and demand in the long-term care industry - the "institutional residential care attendant dimension", the framework for the limitation of social values, the role of development, and the cultivation of talents. Strategies, Institutions and Policy Systems. Interviews with long-term care industry practitioners or senior executives with more than ten years of long-term care experience to discuss the actual demand for front-line care attendants. It is hoped that government policies should be more appropriate and more appropriate in terms of long-term care industry care manpower development planning and replacement manpower regulations. Actively attaching importance to talent diversity and the cultivation of practical skills, industry-university cooperation among inter-professional teams, and my country's emphasis on the correct direction of human resources development strategies in the long-term care industry, can reduce the long-term care manpower gap and increase the retention rate. It is hoped that the manpower of care attendants can meet the needs of long-term care in my country and solve the problem of long-term care production.
