臀部肌群肌力訓練結合肌內效貼紮對髕股關節疼痛患者的影響

The Effect of Hip Muscle Strengthening Combined With Kinesio Taping in Patients With Patellofemoral Pain

摘要


髕股關節疼痛 (PFP) 是成年女性常見的肌肉骨骼相關問題,臀部肌群功能的異常是PFP的病因之一。過去研究指出臀部肌群肌力訓練能改善患者疼痛情形;肌內效貼布 (KT) 常用來處理肌肉骨骼問題相關問題,但KT對肌力、本體感覺、神經肌肉的影響仍存在爭議。本研究將11名受試者隨機分配至3組,3組皆進行相同的六週臀部肌群肌力訓練,但配合上不同的KT情形:無KT、安慰劑KT、臀中肌促進KT。以混合設計二因子變異數分析各組患者前測、3週後測、6週後測的疼痛症狀和功能性表現分數,以及功能性動作中關節活動度、平均肌肉活化程度、肌肉活化時間差。不論組別,經過六週介入,受試者疼痛症狀和膝關節功能性表現顯著進步,側向下階的髖關節內收角度減少,階梯行走中臀中肌延遲活化減少。而階梯上行承重回饋期的臀中肌平均活化程度和階梯上行的活化時間差有交互作用。本研究結果指出六週臀部肌群肌力訓練能有效改善PFP患者症狀,並提升患者對髖關節穩定性和神經肌肉的控制,然而KT對患者的效益仍不明確。本研究建議臀部肌群肌力訓練可作為PFP患者的復健選項,而KT對PFP患者臀部肌肉功能的影響需要更多研究來探討。

Patellofemoral pain (PFP) is a common musculoskeletal problem in adult women, and abnormal hip muscle function is one of the factors of PFP. Previous studies have indicated that hip muscle strengthening programs can improve pain in patients; Kinesio Taping (KT) is used to manage musculoskeletal problems, but the effects of KT on muscle strength, proprioception, and neuromuscular control are still controversial. In this study, 11 subjects were randomly allocated to 3 groups, and all groups underwent the same six-week hip muscle strengthening program, but with different KT conditions: no KT, placebo KT, gluteus medius facilitated KT. A two-way mixed-model analysis of variance (ANOVA) was used to analyze the pain intensity and knee functional scores at the baseline, 3-week test, and 6-week test in each group, as well as the range of motion, the average amplitude of muscle activation, and the muscle onset timing differences during the functional tasks. Regardless of the group, after six weeks of intervention, the pain intensity and knee functional score were significantly improved, the hip adduction angle was reduced in the lateral step-down task, and the delayed onset of the gluteus medius was reduced during stair stepping. The average amplitude of the gluteus medius activation during the loading response phase of stair ascending and the onset timing difference of the stair ascending have interaction effects. The results of this study indicate that six weeks of hip muscle strengthening can reduce pain in PFP patients and improve the stability of the hip joint and neuromuscular control of patients. However, the effect of KT on patients was still unclear. This study suggested that the hip muscle strengthening program was recommended as a rehabilitation for PFP patients. The effect of KT on the hip muscle function of PFP patients needs more research to be confirmed.
