牙齒填補材料複合樹脂存活期限之整合分析

摘要


牙科健保中最常被提及浪費項目是牙體復形重覆填補,依據健保資料指出牙科處置費佔其牙科全國主要醫療費用78.76%,其中牙體復形處置又佔其醫療費用51.18%。因此本研究將討論恆牙、恆牙Cervical 及乳牙等部位用複合樹脂牙科填補物,以整合分析(meta analysis)來探討其存活率。使用MEDLINE 醫藥學文獻資料庫搜尋程式,由文件分析和檢索系統,計算機幫助搜尋牙體復形發表的文章。利用CMA (Comprehensive Meta Analysis) Version 2.0 軟體來進行整合分析。 由結果得知,恆牙複合樹脂充填後存活率及95%CI 第一年為93.3% ( 91.7% - 94.7%)、第二年為92.4% ( 91% - 93.6%)、第三年為90.2% ( 88.8% - 91.5%)、第五年為90.5% ( 89.3% - 91.6%);恆牙Cervical 複合樹脂充填後存活率及95%CI 第一年為87.7% ( 85.5% - 89.6%)、第二年為84.1% ( 80.5% - 87.1%)、第三年為86.9% ( 82.9% - 90.2%)、第五年為83%( 80.7% - 95.7%);乳牙複合樹脂充填後存活率及95%CI 第一年為93.7% ( 91.9% - 95.2%)、第二年為88.7% ( 86.2% - 90.8%)、第三年為89.9% ( 77.6% - 95.8%)。 本研究結果提供國外使用複合樹脂作為牙科填補材料的存活率資料,作為國內健保制度牙科重複填補的給付點數核減與調整的基礎,並評估其合理的『不應再給付期』。關

The most notable for being a waste in dental health insurance is the re-implant for Prosthodontics, according to the health insurance database, a large percentage of dental expenses to national main medical expenses is 78.76%, above all, Prosthodontics expenses come up with a 51.18% to medical expenses. Therefore this report will be discussed the longevity of dental restoration failure rate with meta-analysis for dental filler with composite resin in permanent teeth, permanent cervical teeth, and primary teeth. To access researching in the MEDLINE medical databank, to analysis and to retrieve the article for Prosthodontics. To integrate data with CMA (Comprehensive Meta Analysis) Version 2.0. According to test reports, the survival rate for composite resin in permanent teeth at 95% confidence interval 93.3% ( 91.7% - 94.7%) the first year, 92.4% ( 91% - 93.6%) the second year, 90.2% ( 88.8% - 91.5%) the third year, 90.7% ( 88.9% - 91.2% ) the fourth year, 90.5% ( 89.3% - 91.6%) the fifth year. The survival rate for composite resin in permanent cervical teeth at 95% confidence interval 87.7% ( 85.5% - 89.6%) the first year, 84.1% ( 80.5% - 87.1%) the second year, 86.9% ( 82.9% - 90.2%) the third year, 83%( 80.7% - 95.7%) the fifth year. The survival rate for composite resin in primary teeth at 95% confidence interval 93.7% ( 91.9% - 95.2%) the first year, 88.7% ( 86.2% - 90.8%) the second year, 89.9% ( 77.6% - 95.8%) the third year. This report works as a backgrounder for the longevity of dental restoration failure rate overseas in taking composite resin as dental filler, to be treated as a benchmark to help audit the fee schedule, reappraise the rational of health care fee point.
