下顎快速立即受力植牙之病例報告

Immediate Loading and Teeth-in-an-hour Implantation-Report of Four Cases

摘要


人工植牙發展已有一段時日,由於材料的改善、兩階段手術觀念的提出以及精細之手術操作技巧,在臨床使用上有相當高的成功率。但兩階段式的人工植牙的缺點是治療前後費時甚長,亟待克服。根據簡化植牙手術及減少治療時間的思維,目前植牙之觀念已朝向一次手術、立即膺復的方向發展。本次報告我們提出四個下顎快速立即受力植牙病例,經由術前縝密的檢查及制定治療計畫、電腦斷層定位、3D立體模型建立及精密手術導引板製作,在一小時之內,手術及下顎覆蓋式義齒贋復同時完成。手術無需切線、翻瓣及縫合,患者術後之腫脹疼痛較傳統植牙手術輕微甚多,且術後即可正常嚼食,病患相當滿意。此種植牙技術與傳統植牙手術有相當大的差異,本報告將提出四個病例,說明此種植牙設計與適應症,並概略說明手術之步驟及過程。

The use of dental implants in rehabilitation is an established technique that has been used for a long time. Improvements in materials, a two-stage surgical protocol and skillful surgical techniques make dental implants a reliable treatment modality with a high success rate. However, the long treatment time is a major disadvantage to the two-stage implant protocol. Modern implant treatments aim toward a one-stage surgery and the immediate loading of the prosthesis to shorten the treatment time and simplify the procedure. In this report, four cases of immediate loading implantation of an edentulous mandible are presented. The application of careful physical examinations and treatment planning, implant location with CT scan, 3-D model replication and a precise surgical stent allow the surgery and prosthesis to be completed within an hour. In addition, no incision, flap reflection or suture is needed during surgery. Further, the mandibular overdenture can be loaded immediately after surgery. The goal of this report was to present the teeth-in-an-hour protocol as another treatment modality of the edentulous mandible.
