Clinical Application of Sustained Low Efficiency Dialysis in Critical Acute Kidney Injury

摘要


持續性低效率透析(sustained low efficiency dialysis, SLED)是一種融合式透析治療方式,設定較緩慢的血液與透析液流速,經過較長透析時間的治療來達成合理的透析結果。既可如同連續式腎臟透析(continuous renal replacement therapy, CRRT),維持重症急性腎損傷患者穩定的血行動力變化,達成合理的水分移除及足夠的毒素清除率目標,也可避免耗費過多醫療資源與成本花費。雖然仍未被證實比CRRT更能改善重症急性腎損傷患者存活率,也必須克服護理人力不足的難題。但SLED擷取了傳統間歇式血液透析(intermittent hemodialysis, IHD)與CRRT兩者的優點,是治療急重症患者透析方式的合宜選項。

Sustained low efficiency dialysis (SLED) is a hybrid mode of dialysis characterized by reduced the rate of dialysis and prolonged dialysis duration to achieve reasonable dialysis targets. In critical acute kidney injury patients, SLED has proven to possess advantages of both continuous renal replacement therapy (CRRT) and intermittent hemodialysis. SLED can maintain hemodynamic stability during each hemodialysis session and result in successful fluid balance and efficient clearance of small solutes. SLED can reduce the cost of dialysis treatment compared to CRRT. Among randomized controlled trials, there is no difference in mortality rates between SLED and CRRT. SLED has to overcome the shortage of trained dialysis nurse staff in current medical environment. SLED is considered to be a viable dialysis option in the treatment of critical acute kidney injury patients.
