使用細孔洞鈦網和膠原再生膜重建失敗植體區的骨質再生:病例報告

摘要


人工植牙技術引進國內已有十年以上歷史,雖然其長期臨床成功率有90%以上,然而植體因咬合受力不當、感染等各種因素拔除的,相信也不在少數。其中尤以因感染移除的失敗植體區,因骨頭破壞吸收嚴重,最不易重新植牙贗復。本病例報告的兩位患者均是因植體失敗,在植體移除後留下一大而深的碗型骨破壞。為重建骨質再生,選擇以細孔洞鈦網維持空間合併膠原再生膜穩定血塊﹕六至九個月後,移除鈦網時,已可見良好堅硬之新生骨形成,同時順利植入新植體贗復,因此提出報告﹕由本案例可發現在不放任何骨填充物的情況下,單單倚靠細孔洞鈦網和膠原再生膜穩定血塊,即能達成預期的引導骨質再生效果。

Dental Implants had been introduced to Taiwan for more than ten years. Although the long term clinical success rate is over 90%, we believe that there are lots of cases that implants extracted due to improper occlusal force, infection etc. Among these failed implants, the implants removed due to infection are hard to reimplantation due to severe bony destruction. Two implants in this cases report were removed due to failure. Large and deep saucerization bony destruction were left. For bony regeneration, we chose micro-titanium mesh to maintain space combined collagen membrane to stabilize blood clot. 6 to 9 months later, we got new bone formation when removed the titanium mesh, and reimplantation successfully. In these cases, we can have a predictably guide bony regeneration result without any bone graft.
